Dear Mr. McKinney:
On October 11, 2006, the Region I Field Office, NRC Office of Investigations (OI), initiated an investigation (Case No. 1-2007-004) to determine whether a maintenance foreman had a refueling worker move the refueling platform without a second qualified operator or a qualified spotter, whether licensee management allowed the refueling platform to be parked in an unauthorized location, and whether licensee management allowed the refueling platform to be parked with items hanging from the hoists, all in violation of station procedures.
Based on testimonial and documentary evidence developed by OI during its investigation, OI did not substantiate that a maintenance foreman or PPL management directed or allowed station procedures to be violated. Additionally, the investigation did not support that there were any violations of station procedures associated with the above-cited concerns.
