Fwd: Vogtle Electric Generating Plant - Unit 2, Results of Reactor Pressure Vessel Head Inspections Greetings, I would like to arrange a phone call with the Vogtle licensee about their recent reactor vessel head inspection last outage (see attached email). There is no near urgency, but I would like to schedule a meeting at least in July or early August sometime. The purpose of the call would be to discuss two items; first is a question about the inspection, and the second is about the thermal sleeve wear issue.

In the question about the inspection, one line in the attached report raises some questions.

"There was one surface geometry indication identified during the eddy current reactor vessel head vent line penetration examination." Operational experience with primary water stress corrosion cracks in weld materials has shown that they may have very small surface breaking indications. Below are examples of the types of questions I would ask;

- Was this an inspection of the partial penetration weld?

- What was the size of the indication?

- How was depth verified?

- By "surface geometry indication" was this being characterized as a scratch or grinding marks?

- What other actions were taken to verify this was not a flaw?

- Was a bare metal visual inspection performed around the vent line penetration?

On the thermal sleeve issue, thermal sleeve wear to the extent that the sleeves need to be removed is a new item for which we do not have a general inspection requirement. As far as the concern, it would be more of a loose parts issue or the potential to cause a problem with a control rod's movement. Since Vogtle appears to have a plan to address this, we would be looking to gather information as to if this should be a general concern.

Thanks for your time and effort in setting up the teleconference. If possible, before the phone call, it would be useful to have any diagrams that could better explain both the inspection finding and the wear on the thermal sleeves. If you have any questions or comments, please let me know.
