2006

  • Successful Unit 1 Refueling Outage
  • Completed Phases 1, 2 & 3 of B.5.b NRC Security enhancements
  • Material Condition Improvements

- DEH

- Alloy 600 inspections and upgrades on unit 1

- Feed Water Regulating Valve Digital Positioners

- Four new air-cooled Station Air Compressors

- 1D and 2C RCP Motor replacements

- Fuel Transfer System reliability modifications

Performance Issues

  • NRC Performance Indicators

- All indicators are Green with the exception of AF Heat Removal. The Unit 2 indicator is white due to three 2B AF functional failures in March and November 2005 and January 2006. Expected return to green by April 2008.

- NRC Supplemental Inspection of Unit 2 AF white indicator conducted in January 2007 with no findings of significance identified and adequate corrective actions.

Cross-Cutting Human Performance (HU)

  • Initial concern noted in August 2005 Mid-Cycle Assessment letter
  • Identification as a cross-cutting issue in NRC March 2006 End-of-Cycle Performance Review letter
  • Corrective Actions:

- Established a dedicated Human Performance Coordinator position reporting directly to the Plant Manager

- Established Site Human Performance Leadership Committee

- Conducted a detailed common cause analysis of cross cutting issues and actions developed to address:

  • Supervisory oversight and engagement
  • Procedure quality

- Developed Site Wide Human Performance Improvement Plan (HPIP)

  • Incorporated existing Departmental Human Performance Plans
  • Incorporated actions from Common Cause Analysis
  • Obtained input from workforce through Human Performance Review Panel
  • Progress monitored through HU Leadership Committee

- Improvement noted in performance
