  • Sample was collected by a supplemental plant employee being trained as a collector for outage assistance. The collector section of the custody and control form (CCF) was not completed, resulting in a fatal flaw (lab refusing to analyze). This necessitated a re-collection. This issue was entered into the corrective action program and the training procedure revised to require the trainer to independently review all CCFs prior to the trainee sealing them in the bag to transport to the lab.
      • 1 sample was a re-collection of a pre-access sample that the lab was unable to analyze due to amino acid interference. When the sample was re-collected the collector observed it had an oily sheen.

Upon consultation with the MRO, the sample was immediately re-collected as an observed sample.

1 sample was re-collected due to a dilute follow-up test.

All persons testing positive were denied access. Their supervisor and access requestor were notified.

NOTE: A short term contractor tested in the 0.01-0.039 range (Confirmed @ .0385) during a pre-access test. The contractor confirmed the test was conducted after his mid-shift break. Per Ginna procedure "Management Sanctions" the test was treated as a positive test since it occurred after the mid-shift break. Access was denied.

A supervisory review was conducted for each non-negative test noted above where the individual had been within the protected area either as a visitor or with unescorted access. None of the supervisors had concerns with the employee's work performance. None had performed work in vital areas. No remedial actions were required.
