Nolan Other RemarksComnentsLssons Learned: 01 report found that individual, a general foreman, delberately failed to take action after two kindivkials reported the odor of alcohol on a foreman. The call was made based on the No welders report that the incident had happened backed by ft staternent from three other foreman that the incividual had told them after the fact that the welders had come to hirm The indMdual told boti the licensee and 01 that the incident had never occurred. There is a need with this case to question the exdstence of a chilled environment as a result of the two wekders being laid off for not reporting the FFD concern.
