PSEG Dave and Eileen, The following is a copy of an email sent out at PSEG Nuclear on Thursday, October 23.

While I do not believe it conveys the view of the outage manager or site outage manager, I do believe it gives great insight into the current production culture at the site. No matter who wrote it or how it got sent out without adequate management review, it signals a "paradigm" that I believe is inconsistent with "Safety First." To my knowledge, no correction has been sent out.

Mid-Outage Safety Stand Down Roll-out~

Importance: High Due to some confusibn I am sending this email to provide supervisors/managers with a copy of the Mid-Outage Safety Stand down-presentation that will be held tomorrow (1'0/24). This isa result of numerous incidents that have resulted in safety concerns, tagging and OSHA recordables. The Safety Line of Business Council, Management Representatives,. Safety Department, and the Outage Group have meet frequently over the past week to address these issues.

This will be conducted by Supervisors or their designee. The duration is an hour long, with time for discussion. Please coordinate your rollouts appropriately as to not hinder our outage schedule. Thi~s is a mandatory Mid-O'utage Safety Stand down. Attendance will be captured in SAP. The presentation come complete with Presentation Guide for those rolling this information out.
