My suggestions:
- a. The general layout is good. I would add an additional column reflecting our current assessment of safety impact for each item based not only on what we are getting out of the interviews, but on what we already know about the issue (i.e., through inspection effort, NRC staff knowledge).
- b. I would also include all of the discreet (non-touchy-feely) issues in the list, however vague they may be, just so we can swy we assessed them, i.e.
- excessive use of temp logs
- Salem 2 IS[ relief request re: piping UIT (coverup?)
- "offgas" issue @ HC after Anderson took over; rad safety concerns expressed but not resolved
- HC employee asked to modify a Notification re: "in-leakage" (should be fleshed out during the inteview with that individual) 7)
