- erview held on 10-22-03.

Page 7 - When asked if he had been subjected to any sort of management pressure... 'let's get the job done and forget about safety or look the other way or well worry about safety next time' he responded with "... never do'ne that with me. I have heard of instances, but I have never seen it and never experienced it."
Page 7 - Affirmed that he has stopped a job before in the past to ensure industrial safety Page 8 -"[Industrial safety numbers] are going in the wrong direction" Page 8 - "I have nothing In the nuclear safety issue" Page 9 - "For nuclear safety, I would say they [the general flow of things... getting better/worse] are improvingff Page 11 - Chronic problem with Boric acid transfer pump seal leakage Page 11 - Chronic problems with the 23 and 13 C 'harging pumps... NEEDS TO BE DEVELOPED MORE (what problems or Issues exist, have condition reports been written, etc.)
Page 12 - Controller problems with the "CC unit chargers for some time" Page 12 - "There's a lot of repeat work" Page 13 - Greater than. 5 years ago procedural performance expectations led t o multiple reverse power trips on EDGs.
(Incidently... still have 1NEO! manipulating the components to synch and load the edg.)
Page 17 & 18 - ýSWitchgear ventilation is where we have the majority of the problems with the high D/Ps... They Identified a damper problem nine months ago that had to do with some DP somewhere" 0
Page 20'- "[MS52s]... we have had problems wi.th [EDG] governor oVer-speeding... replaced quite a few governors on the pumps" '.. When asked if engineering got Involved -"~Initially, no, but after a couple, yes... lately they seem to be coming on boardl a lot earlier"'
Page 23 - "As equipment operators), I feel like we Identified the [boron) leakage on a flange many years before It became a real issue... When it became an Industry-wide problem... carbon steel bolts being eaten away, somebody walked down there and said We have a major problem... we had been telling them that.... It took somebody else..

Page 24 through 26 - Good discussion about a sense of 'taking the easy/cheapest way out' - multiple bolt replacements for the above boron leakage problem & charging pump cooler cleaning during the 'grassing season' Page 29 - On surveys: "They do It [take surveys] so that they can get numbers to justify whatever they are justifying.
Ihave not seen a lot of correction come from the survey... On my particular shift, there were a lot of actions."

Page 32 & 33 - "... I have gotten to the point where I deal strictly with my immediate supervision... I have gone higher... there's no need to go any higher because of the change...
I don't have a whole lot of respect for anybody higher than my supervision anymore... had a lot of personal situations In the last couple of years. I have been lied to a lot. I've brought up various Issues that have seen brick walls... has gotten better... still a lot of guys... that will cut corners [on Industrial safety]... I have lost a lot of confidence In the upper management..."
