SYNOPSIS This investigation was initiated by the U.S. Nuclear Regulatory Commission (
)Office of Invstgaios
.I3(RI), on January 8,2004, to determine whether a
- deliberAtey violated a technical specification required plant procedure and/or NRC regii ations on September 22, 2002, by manually closing a valve to isolate' a steam leak without the required approval and/or knowledge of the control room staff.
Based upon evidence developed during this investigation, 01 found that the 4I0vas not a member of the on shift operations crew in the control room on September 22, 2002, when he/she manually closed the valve in question. However, 01 did not substantiate that the Wf illfully violated the technical specification required plant procedure or NRC regulations/ requirements when he/she closed the valve.
