??s EA-04-131 Cooper CASE TOPIC - COOPER - EA-04-131 - Failure to properly restore the service water to its normal configuration following routine maintenance in that the licensee did not realign the gland water following service water maintenance. The failure to realign the gland water was attributed to inadequate instructions. The inadequate instructions led to the cross connection of gland water resulting in a loss of service water redundancy.
Last I Know - The last I know of this case is from a strategy form that was written by Doug Starkey on 02/03/05. The strategy form stated that there would be one GREEN finding and violation of Criterion V for inadequate written instructions for restoring service water to operable status following maintenance. The strategy form also indicated that there would be an NOV rather than an NCV. The last Region IV status I have (02/14/05) indicates that the Region plans to issue to issue Green finding/NCV.
QUESTIONS - Is this supposed to be a GREEN finding/NOV or GREEN finding/NCV? What's the status??? - Has this package been issued yet?
