OE Lessons Incorporated into Actual work - A Success

  • ANO: Safety Initiative:

- Prior to ANO Unit 1 steam generator replacement outage, an event at another station occurred with the same type of lifting device that ANO was using to move the steam generators. The event involved some hydraulic hoses to the wedge assemblies reversed during installation because the hoses had no identifying marks. This led to all the lifting strand wires falling out of the strand jack and landing on the reactor cavity deck approximately 100 feet below 7
