Davis-Besse Assessment Results Jan 1 - Dec 31, 2005 Davis-Besse was in the IMC 0350 Process for the 1st and 2nd Quarters of CY 2005 On July 1, Davis-Besse transitioned from the 0350 Process to the Reactor Oversight Process

  • Regulatory Response Column due to an open White finding in the Emergency Preparedness Cornerstone A 95001 supplemental inspection was performed in October 2005 to review the open White finding No substantive cross-cutting issues identified www.nrc.gov

Safety Significant Findings or PIs A White finding was issued in 2Q2005 in the Emergency Preparedness Cornerstone. The finding and violation related to the testing of alert and notification system sirens in May 2004. The finding was closed in 4Q2005.

The NRC declared the alert and notification system performance indicator (ANS-PI) data for 2Q2004 and 3Q2004 discrepant. As a result the ANS-PI crossed the green-white threshold and was white in 2Q2004.

The ANS-PI returned to Green in 2Q2005.

Davis-Besse Inspection Activities Jan 1 - Dec 31, 2005 Approximately 2200 baseline inspection hours More than 500 additional inspection hours 14 total findings and violations Order-related mid-cycle outage (Jan. 17 - Feb. 10) to inspect reactor vessel upper and lower heads and steam generator tubes www.nrc.gov

Davis-Besse Inspection Activities Jan 1 - Dec 31, 2005 NRC issued a Notice of Violation documenting nine proposed violations and a $5.45 million civil penalty on April 21, 2005 Problem Identification & Resolution inspection (in addition to routine biennial inspection) in August 2005 NRC review of Order-related independent assessments

  • Operations Performance, Engineering Effectiveness, Safety Culture / Safety Conscious Work Environment, and Corrective Action Program Effectiveness www.nrc.gov

Davis-Besse Annual Assessment Summary Jan 1 - Dec 31, 2005 FENOC operated Davis-Besse in a manner that preserved public health and safety All cornerstone objectives were met with one White finding (EP performance deficiency regarding siren testing) that was closed in 4Q2005 NRC plans inspections in CY2006 to review the results of the four independent assessments www.nrc.gov
