Dear Mr. John Petro:
This constitutes a Violation Notice pursuant to Section 31(a)(1) of tb* Illinois Environmental Protection Act. 41S IJCS 5/3 L1a)(I), and is based upon review of available infonnation and investigation by rcprrsentatives of the Illinois Enviromnnental Protection Agency ("Illinois EPA").
The Illinois EP.A hercby provides notice of violations of environmental statutes, regulations or permits aq set tlfh in Attachment A to this letter. Attachment A Includes an explaration of the setivIties that tho Illinois EPA believes mayrcsolvc the specified violations. incluhing an estimate of a reasonable time period to complete the necessar.Y asWivities. Howevtr, due to the nature and
.seriousness of the violations cited, please be advised that resolution of the violations may also
