Root cause evaluation received from Lochbaum On Tuesday Dave Lochbaum forwarded a PSEG, SL 1 root cause evaluation of global organizational and programmatic issues. The attached file is the residents' review of the context and conclusions of this root cause evaluation, which broadly addressess problems at Salem and Hope Creek, and which is draft and under management review.
Overall, I believe the evaluation does not raises any new issues, it does not merit separate regulatory response actions now, and PSEG is likely fold it into their ongoing evaluation of work environment concerns.
Thanks to Mel and Marc for reviewing this. I believe the review is done for now. We will evaluate its role in future review efforts.
