potential fitness for duty issue Late in the shift on Wednesday October 5, 2005, Dave Werkheiser was in the control room observing Ginna's quarterly TDAFW IST. Dave noticed that the onshift STA appeared to have trouble staying awake (sitting still in corner, nodding head). Dave noted that other onshift personnel and a QA evaluator appeared aware of the STA's inattention but did not take action to ensure the STA remained alert. Dave presented his concern to the shift manager, who promptly engaged the STA and commented to Dave that he planned to counsel the individual. Next steps: Ensure Ginna SRI/RI informed. Discuss with Larry Doerflein first thing in morning. Hold off sharing observation with Ginna management until discussed with Larry. Ginna CR?
