ES-401 Record of Rejected K/As Form ES-401-4 Randomly Selected Tier / Group Reason for Rejection K/A

1. 6 Cannot write an Emergency Plan K/A against this EAPE. Replaced RO 1/1 022 2.4.50 with examiner concurrence with 027 2.1.20.

1. 11 Unable to write an acceptable question on this K/A. Randomly RO 1/1 056 AA2.42 replaced with 056 AA2.17.

1. 15 SRO-only K/A as involves selection of procedures, not appropriate RO 1/1 E04 EA2.1 for RO exam. Randomly replaced with E04 EA1.1.

1. 20 PI procedurally is not allowed to operate with a dropped rod, so RO 1/2 003 AK3.07 unable to write a TS question on this operation. Randomly replaced with 067 AK3.04 as no K3 level K/A's could be used for this APE.

1. 22 PI CR evacuation places SRO's in locations where SG level is RO 1/2 068 AA2.01 determined and acted upon. Randomly replaced with 068 AA2.07.

1. 33 Unable to write question on this K/A due to plant method of drawing RO 2/1 007 K5.02 a pressurizer bubble. K/A previously rejected on 2004 exam. Randomly replaced with 013 K5.02 as no other 007 K5's are above 2.5.

1. 40 Equipment not installed at PI, replaced with 039 A3.02 (randomly RO 2/1 026 A3.02 selected system as no other A3 K/A available for system 026)

1. 50 Radiation effluent releases for PRM already part of NRC JPM exam and similar to other questions (i.e. 068 K6.10) on NRC and audit. No RO 2/1 073 K3.01 other 073 K3 K/A's exist. First randomly replaced 073 K3.01 with 078 K3.03. This resulted in no 073 system selected, so replaced duplicate selected system K/A 063 A4.03 with 073 A4.03.

RO 2/1 063 A4.03 Replaced with 073 A4.03 per above.

1. 53 Instrument air not TS related equipment at PI, replaced with 059 RO 2/1 078 2.2.25 2.2.12 (randomly selected system as no other G 2.2 K/A's available for system 078).

1. 55 Emergency Containment Entry JPM is part of audit exam, unable to write question that is not repetitive. Replaced with 007 A3.01 as 103 RO 2/1 103 A2.05 A2.04 is already selected and system 007 was only 2/1 system not already selected. A3.01 selected with A3 category chosen as only A3 K/A and Tier 2 only had 2 A3's previously selected.

1. 77 Two 027 EAPE K/As randomly selected initially, this one replaced SRO 1/1 027 AA2.10 as similar to audit scenario malfunction. Randomly replaced with 038 EA2.02.

1. 81 Cannot write SRO level question on this K/A. Randomly replaced SRO 1/1 057 AA2.18 with 005 AA2.01.

1. 82 Cannot write SRO level question on this K/A. Randomly replaced SRO 1/2 024 AA2.04 with 069 AA2.01.

1. 84 Cannot write SRO level question on this K/A. Randomly replaced SRO 1/2 060 AA2.03 with 037 2.1.2.

1. 86 Unable to write an acceptable SRO level question on this K/A.

SRO 2/1 004 A2.19 Randomly replaced with 004 A2.06.

1. 91 WD program did not include Fuel Handling System 34 K/A's as required by Rev 9. Per discussion with Chief Examiner on 1/19/05, randomly replaced one Tier 2/Gp 2 SRO System with randomly selected SRO 2/2 011 A2.04 fuel handling system K/A 034 A2.02. A satisfactory question could not be written on this K/A due to limited exposure/training of plant SRO's on cask handling. Substituted 034 A2.03 as A2.01 only other choice and it was too similar to #8.

1. 100 Cannot write SRO level question on an RO task K/A. Randomly SRO G 2.4.34 replaced with 2.4.1.
