Primarily refers to tests conducted for samples that were hydrated (diluted). A second (observed) sample is conducted following results indicating low specific gravity and/or creatinine.

Situation Descriptions

1.

One employee was confirmed positive for Opiates as a result of a follow-up drug and alcohol test.

2.

One employee was confirmed positive for Alcohol as a result of a random drug and alcohol test.

3.

Four individual Contractors were confirmed positive for THC as a result of pre-access drug and alcohol testing.

4.

Two individual Contractors were confirmed positive for Cocaine as a result of pre-access drug and alcohol testing.

5.

One Contractor was confirmed positive for Cocaine as a result of a for-cause drug and alcohol test.

6.

One Contractor was confirmed positive for Cocaine and Alcohol as a result of a for-cause drug and alcohol test.

B.

Manaeement Actions (in Response to Situation Descriptions)

The following actions were taken by management as a result of the above situation descriptions:

1.

Unescorted access was immediately suspended on one (1) employee and eight (8) contractors for positive drug test results. Each for an indefinite period of time.

2.

Unescorted access was immediately placed on hold for one (1) employee that was alcohol positive. Individual wvas placed in rehabilitation for substance abuse. Upon return individual was put in follow-up testing program and is being followed by site Employee Assistance Program (EAP).

3.

Substance Abuse Referrals:

All individuals were encouraged to obtain a substance abuse evaluation to determine proper treatment and rehabilitation.

4.

Review of safety-related work was performed where applicable. No remedial actions were required.

There were no reportable events during this performance data period.
