2 MR. PARTICIPANT: I'm just calling you to 3 notify you of an alert of one of our plants that we 4 received today.

5 MS. DOYLE: Okay. Okay.

6 MR. PARTICIPANT: And the name of the 7 plant of Millstone.

8 MS. DOYLE: Okay.

9 MR. PARTICIPANT: Okay. We received 10 notification at 0923 and they declared the alert at 11 0842 this morning.

12 MS. DOYLE: Okay.

13 MR. PARTICIPANT: And what happened there 14 is that they had one of their valves on their 15 secondary system that opened unexpectedly.

16 MS. DOYLE: Um-hum.

17 MR. PARTICIPANT: And then -- but, now, 18 they're reclosed and the system -- and they're trying 19 to stabilize the system. The unit -- the reactor 20 tripped on low pressure and they're in the process of 21 recovering from a safety injection.

22 At the moment, we don't think that there 23 is -- they don't think there is any security breach.

24 They're following up and they're trying to verify on 25 that.

22 1 MS. DOYLE: What do you mean by security 2 breach?

3 MR. PARTICIPANT: Well, because, you know, 4 if -- if those valves lifted and -- unexpectedly. So, 5 someone is looking into it. So, since we know that 6 you'd be concerned about that question, we're trying 7 to give you a heads up that at the -- at this moment 8 they don't think that it is a security problem.

9 MS. DOYLE: They just think it's a 10 malfunction?

11 MR. PARTICIPANT: Yes, it could have been.

12 Yes, they're in the process of looking into it. So, 13 the plant is shut down and they're trying to --

14 they're stabilizing it right now. So, but there's no 15 other major issue right now.

16 MS. DOYLE: Is this a big deal or is this 17 a fairly small --

18 MR. PARTICIPANT: Well, alert -- alert is 19 -- is a -- is -- our level of declaration go from a --

20 from an unusual event and the next one would be an 21 alert. So, it is important. They're looking into it, =

22 but like I said, the -- the valves reseated back and 23 so, they're all closed again and the plant is stable.

6 MS. DOYLE: All right. And Millstone is 7 where? Is that Virginia?

8 MR. PARTICIPANT: No, Millstone is in --

9 is in Connecticut.

10 MS. DOYLE: I knew it sounded familiar.

11 Millstone. What town in Connecticut?

12 MR. PARTICIPANT: Okay. Give a second.

2 Now, they are now reclosed and the system 3 is being stabilized right now, the plant, and 4 obviously, the plant tripped. So, the reactor tripped 5 in low pressure and they're in the process of 6 recovering from a safety injection.

7 Now, the -- our Agency entered what we 8 call a monitoring mode at 0948.

9 MR. SMITH: Are you going to send us 10 something on this?

11 MR. PARTICIPANT: Yes, we are. So, we're 12 in the process now of writing it up and we'll send it 13 to you and we'll also keep you up to date if the 14 status changed from what we just told you.

15 MR. SMITH: Okay. Was there any type of 16 leak? Anything, sir?

17 MR. PARTICIPANT: No, you mean radioactive 18 leak? No. No, it was basically steam on the 19 secondary side of the plant.

20 MR. SMITH: Okay. Your name again please?

12 MR. COSTA: Okay. And emergency 13 declaration was an alert and they declared that at 14 0842. They notified us at 0923 and our Agency went 15 into what we call our monitoring mode at 0948 and 16 here's the --

17 MR. BISCOE: Went into the monitoring mode 18 at what time?

19 MR. COSTA: 0948. And here's just a quick 20 summary for you.

21 MR. BISCOE: Okay.

22 MR. COSTA: So, that was because of one of 23 our -- one of their -- their safety valves on their 24 secondary side of the plant opened up unexpectedly.

29 1 actually earlier, those valves closed back up and the 2 plant is, of course, shut down. The reactor tripped 3 and plant is in the process of being stable right now.

4 They're recovering from what we call a safety 5 injection.

6 And in case anybody asks, the plant has 7 their -- we're almost sure that this has nothing to do 8 with any security breach or anything like that, but 9 they're verifying that also in case someone asks you.

10 MR. BISCOE: Okay. All right. It was the 11 safety valve on secondary side of plant opened.

12 MR. COSTA: Unexpectedly. Important to 13 write that there and -- and that now, which is a 14 little while ago, they were reclosed and the system is 15 stabilizing.

16 MR. BISCOE: Okay. And it's now 17 stabilizing?

18 MR. COSTA: Right and the plan obviously 19 tripped and the reactor tripped on low pressure.

20 Anything else?

21 MR. BISCOE: No.

7 MR. COSTA: They called us early today at 8 0923 to notify of an alert at their site and they 9 declared an alert at 0842.

11 MR. COSTA: Okay. And our Agency went 12 into what we call our monitoring mode at 0948.

14 MR. COSTA: And let me give you just a 15 quick briefing there on what happened.

16 One of their -- several of their safety 17 valves lifted on their secondary side of the plant.

18 They opened unexpectedly.

19 Now, they are reclosed. Actually, it was 20 a little while ago and the system is being --

21 stabilizing right now and because of this valve 22 lifting, the plant tripped. The reactor tripped on 23 low pressure and as part of the recovery from safety 24 injection, they're in that -- in that process right 25 now.

4 MR. HOGAN: Okay.

5 MR. COSTA: And the level was an alert at 6 0842.

7 MR. HOGAN: Um-hum.

8 MR. COSTA: And then our Agency went into 9 what we call a monitoring mode --

10 MR. HOGAN: Okay.

11 MR. COSTA: -- at 0948 and let me give you 12 a quick summary of what happened and obviously, if 13 these things change, we'll call you back --

14 MR. HOGAN: All right.

15 MR. COSTA: -- with an update.

16 MR. HOGAN: Okay.

17 MR. COSTA: So, basically what happened 18 was that one of several main steam safety valves --

19 safety release valves in the secondary system opened 20 unexpectedly --

21 MR. HOGAN: I imagine that woke them up.

22 MR. COSTA: -- and because of that 23 obviously, the plant trips and they tripped on low 24 pressure, but --

2 MR. COSTA: That was an automatic reactor 3 trip. So, right now, the reactor is tripped and the 4 plant is being stabilized right now because it goes 5 through a transient, but there were no other incidents 6 that increased -- changed that. So, they're in the 7 process of stabilizing the plant.

8 MR. HOGAN: Okay. So, this was the main 9 steam safety relief valve on the secondary system?

10 MR. COSTA: Right.

11 MR. HOGAN: Okay.

12 MR. COSTA: That opened unexpectedly.

13 MR. HOGAN: All right. Okay.

14 MR. COSTA: And because of that, they have 15 to declare this alert.

16 MR. HOGAN: Certainly.

17 MR. COSTA: Okay. And -- and --

18 MR. HOGAN: That's right.

19 MR. COSTA: -- obviously, the plant 20 tripped like I said.

21 MR. HOGAN: Right.

22 MR. COSTA: They're -- they're recovering 23 with their -- on their safety injection.

24 MR. HOGAN: Okay.
