9 Now, I know that Diane is listening in 10 from Public Affairs, have we heard back from Bores yet 11 on whether he's made his required notifications or 12 whether he needs some help with that?
13 MR. LANNING: We haven't, Jim. Diane was 14 going to get in touch with the communications lead 15 from Millstone, to be sure that she understood what 16 they knew and what their message was. I'm not sure 17 she's back on our bridge yet from that.
18 MR. WIGGINS: Okay.
19 Bob Bores we haven't heard from.
20 MR. McGINTY: Jim Wiggins, this is McGinty.
21 In reviewing the procedures that we have for declaring 22 monitoring, notifications of Bill Outlaw and Eliot 23 Brenner are on there, so that's an activity that we'll 24 want to track and make sure that occur.

2 We need help to confirm the status of the external 3 notifications by the Headquarters Operations Officer, 4 DHS and FEMA, et cetera.
5 MR. COLLINS: Yeah, Jim, I would suggest 6 that if Bob Bores is out of pocket that we consider 7 getting somebody in there, to get somebody to resolve 8 to manage these calls out of the Incident Response 9 Center.
10 MR. WIGGINS: Yeah, if Bob said he was on 11 his way to the airport, that kind of puts him out of 12 pocket in this, so we ought to try to separately find 13 out where - or at least get another one of the 14 government liaison folks working. The next in line 15 would be Duncan White (phonetic). He looks like he 16 has a New Jersey home address, maybe then it would be 17 Sherry Minnick (phonetic), who has Pennsylvania home 18 address.
19 MR. HOLIAN: Jim Wiggins, Brian Holian, my 20 phone got cut off. I'm back on now.

5 MR. COLLINS: And then, so that we 6 understand the status of the call. If we can't, then 7 we'll just have to try to do them again, and be sure 8 that people have been notified. If they are notified 9 twice, then that's our issue, but we have to be sure 10 they have been at least done once.

12 MR. COLLINS: Wayne, are you monitoring 13 the line?
14 MR. LANNING: Yes, Sam, I'm here, go ahead.
15 MR. COLLINS: Okay, are you comfortable 16 with that approach, to state and governmental 17 notification? You haven't heard from Bob Bores yet, 18 have you?
19 MR. LANNING: I just reached Bob Bores on 20 the phone, we are talking to him as we speak.

22 MR. LANNING: I've also asked for Sherry 23 Minnick to start coming into the IRC.

5 1 Okay, we'll let you manage it on that end, 2 though, as long as you feel comfortable you have the 3 resources and notifications are being made, then 4 you've got the lead on that.
5 MR. LANNING: Okay. I understand I've got 6 the lead for the state, so as soon as I get some more 7 communicators we'll have the communication bridges 8 established.
9 MR. WIGGINS: All right, and when Diane 10 Screnci calls back in, we will get you on the line if 11 you are not on the bridge, and be sure that you are 12 aware of her activities, too.
13 MR. LANNING: Okay, that's good.
14 MR. COLLINS: Jim, at some point we may 15 want to establish a separate bridge for Brian to be 16 talking to Wayne about any follow-up activities that 17 we might want to consider, either in the short term or 18 in the -
19 MR. WIGGINS: See, well, we've got to move, 20 Sam, toward a communication arrangement where we've 21 got a reactor safety link up, an ENS bridge, and then 22 a management link. We've got to move the licensee 23 over to one of those other ones, so we can have those 24 conversations you are asking for.

6 1 Perhaps we can do that when Wayne takes 2 the lead, would that be the right time to do that?
3 MR. WIGGINS: He's working on setting up 4 those bridges in the Incident Response Center now.
5 Yeah, we should do it as soon as the Wayne 6 gets a communicator who could sit and talk to the 7 licensee, you know, maybe we can have the licensee at 8 least kind of trickle him down, and have the licensee 9 move to the reactor safety line, because, you know, 10 until we get someone in the station the licensee is 11 our information tie to what's happening at the plant.
12 So, as soon as Wayne gets a reactor-type 13 communicator in, we should put a safety bridge, a 14 reactor safety bridge up, and get the licensee moved 15 to that, so that we can just have the manager bridge 16 to talk internally about where we want to go with our 17 piece. That's how I would do it.
18 So, Wayne, do you have - when do you think 19 you are going to get a reactor person in?
20 MR. LANNING: Don't know, Jim. We've put 21 out the call, so we are just waiting.
22 MR. COLLINS: I know Don Florek is on the 23 way in, Trap is on his way in, too.

7 MR. WIGGINS: Wayne, do you feel 8 comfortable with the reactor safety bridge, just using 9 who you have there, either Nancy to man that, so that 10 we can try and get these communications sorted out?
11 MR. LANNING: Negative, Jim. We're still 12 contacting people, Nancy is, so I need a few more 13 minutes.
14 MR. WIGGINS: Okay, so she's busy calling 15 people. All right, so we'll just wait for a little 16 bit.
17 MR. WHITE: Jim, this is John White. I'm 18 going to head into the IRC also.
19 MR. WIGGINS: Wayne, does that sound right 20 with you?

6 MR. WIGGINS: And, Millstone, are you still 7 on the line?
8 MR. RYAN: Millstone Station Emergency 9 Communicator on the line.
10 MR. WIGGINS: Okay.
11 Did you get an understanding of when the 12 Emergency Director could talk to us?
13 MR. RYAN: He is - it will probably be 14 about five to ten minutes.
15 MR. WIGGINS: Okay, then I think what's 16 going to happen in this communications is he'll have 17 to call in separately, because we are about ready to 18 establish a bridge and move you over to talk to our 19 reactor safety people, okay?
20 MR. PARTICIPANT: Understand you are going 21 to move me over to talk to reactor safety.

12 You should have heard from Wayne Lanning 13 that we need to set up a reactor safety bridge, and 14 then a management bridge. This is the management 15 bridge we are now talking on.
16 MR. KNOKE: Okay.
17 MR. WIGGINS: Now, we need to set up a 18 reactor safety bridge. Have they told you that yet?
19 MR. KNOKE: No, I just heard the 20 conversation, but I wasn't given any orders to do 21 that.

14 MR. COLLINS: Jim, this is Sam, we should 15 probably, when it's convenient, run through the status 16 of the notifications from the Headquarters Duty 17 officer too, so Wayne has the status of whose been 18 notified, who hasn't.
19 MR. McGINTY: Headquarters Operations 20 Officer, are you available?
21 MR. KNOKE: Yes, I am available.
22 MR. McGINTY: Are you in a position to 23 continue continuity with Wayne?

14 1 Collins and Jim Wiggins have requested an update on 2 the status of your procedures - the alert procedure as 3 well as the go into the monitoring procedure. Could 4 you please give us a rundown of the status of the 5 completed actions in that regard, please?
6 MR. KNOKE: Yes.
7 MR. MCGINTY: Generally, it involves the 8 notifications you have completed.
9 MR. PARTICIPANT: Yes, generally they 10 involve the notifications that you have completed.
11 MR. LANNING: All right, Mr. Ryan, it's 12 Wayne Lanning at the NRC IRC, are you prepared to give 13 us a status brief?
14 MR. RYAN: This is Manager of 15 Communications, and, yes, I can give you a status 16 brief at this time.
17 MR. LANNING: All right, would you go ahead 18 and do that, please?
19 MR. RYAN: I'm not sure how specific you 20 want me to be.
21 MR. LANNING: I'd like for you to be very 22 detailed, starting at the very top with the initiation 23 of the event.
24 MR. RYAN: I understand you want me to 25 start at the initiation of the event.

15 1 MR. LANNING: That's correct.
2 MR. RYAN: At approximately 08:29 we 3 received a reactor trip and a safety injection signal 4 was generated on the alpha channel. We had at least 5 one safety lift initially. It re-seated, and then we 6 had two steam generator safeties- I'm sorry, steam 7 generator safeties lifts the second time.
8 The operating crew entered EQ (phonetic),
9 which is a reactor trip safety injection response, and 10 then transited to ES1.1, which is SI termination.
11 After SI was terminated, we started looking at 12 parameters. We did have primary safety lift at about 13 2350 pounds, thereabouts.
14 At the present time, the plant is stable.
15 We have the alpha - I'm sorry, the bravo charging 16 system up, charging pump running. We had a leak on 17 the charging valve 3 CSV661, and it is isolated at 18 this time.
19 MR. HOLIAN: Wayne, do you want an update, 20 that leak was on a charging system?
21 MR. RYAN: That's correct, it was a leak 22 from a valve.
23 MR. LANNING: Mr. Ryan, are you there?
24 MR. RYAN: I'm right here.
25 MR. LANNING: Okay, go ahead.

16 1 MR. RYAN: At the present time, that's the 2 status of the plant, except for the fact that the 3 terry (phonetic) turbine did get a start signal, and 4 the terry turbine pump tripped, MS5 closed. We had 5 two motor-driven feed pumps, so we did not attempt to 6 reset it initially, but the terry turbine pump has 7 been reset.
8 MR. HOLIAN: Mr. Ryan, this is Brian 9 Holian, you are breaking up just a little bit. The 10 terry turbine has been reset, is your aux feed 11 supplying feed now?
12 MR. RYAN: One second.
13 Understand, we are feeding on three aux 14 feed water pumps, including the terry turbine.
15 MR. HOLIAN: Understand, feeding on three 16 aux feed pumps.
17 MR. RYAN: That's correct.
18 MR. HOLIAN: Steam generators are steaming 19 where now? The MSIVs are shut.
20 MR. RYAN: The steam generator is - I'll 21 have to get you that information, just a second.
22 MR. COLLINS: Brian, I think when we turn 23 over to Wayne, we need to be sure Wayne has a complete 24 list of, not only the initiating event, I thought I 25 heard something about a primary relief in there.

17 1 MR. HOLIAN: Yeah, that's the first I've 2 heard of that, at 2350 they had a primary lift.
3 MR. WIGGINS: That's a new piece, Sam.
4 MR. RYAN: That's correct, it was not 5 communicated with you immediately on my turn over, 6 that primary relief has reseated.
7 MR. WIGGINS: Yeah, how long was it open?
8 MR. RYAN: I will have to get information 9 from the Control Room as to how long that was open.
10 MR. WIGGINS: Well, let's also find out the 11 - I presume that safety - that primary safety goes to 12 a pressure relief tank?
13 MR. RYAN: That's correct, it goes to the 14 PRT.
15 MR. WIGGINS: Yeah, and is the PRT still 16 intact or is just ruptured?
17 MR. RYAN: I will get you the correct 18 information, I do not know that. I believe that it is 19 intact. I will do a follow up on that.
20 MR. COLLINS: The question about any 21 abnormal releases.
22 MR. RYAN: We did not have any abnormal 23 releases.
24 MR. COLLINS: If John White is in there, 25 then we need to be tracking that statement.

18 1 MR. WIGGINS: Yes, can you give us some 2 more information on the relevant sequence, did the 3 steam generator reliefs or safeties open before you 4 saw the primary relief, or was it the primary relief 5 first, followed by the steam generator?
6 MR. RYAN: I don't have that information 7 right now, so you are asking whether the primary 8 relief lifted first?
9 MR. WIGGINS: Yeah, was the steam generator 10 relief lifted, MSIV closed, and then the primary 11 lifted, or was a it a different sequence?
12 MR. RYAN: Understand, you want the 13 sequence of reliefs lifted, concerning the primary 14 reliefs and secondary reliefs, as well as the MSIV 15 closure?
16 MR. WIGGINS: Yes, please.
17 MR. RYAN: I'll have to get that on the 18 PPC.
19 MR. WIGGINS: Okay, thank you.
20 MR. SCHNEIDER: Hey, Jim?
21 MR. WIGGINS: Yes.
22 MR. SCHNEIDER: This is Max Schneider at 23 Unit 3 Control Room.

3 MR. LANNING: Max Schneider, I just want to 4 know your location.
5 MR. SCHNEIDER: I'm in the Unit 3 Control 6 Room, I can give you a little perspective on the 7 pressurizer reliefs if you like.
8 MR. WIGGINS: Yes, please do.
9 MR. SCHNEIDER: They initiated SIAS, and so 10 one train of SIAS initiated automatically, and then 11 they initiated it manually. When they do that, they 12 are charging to the plant, and they, basically, took 13 the pressurizer solid.
14 MR. WIGGINS: Right, they created the high 15 pressure.
16 MR. SCHNEIDER: Right. That happened after 17 the steam generator safeties lifted.
18 MR. WIGGINS: Yeah, okay.
19 Well, Ellis was right then, he said that 20 that was likely what would happen, didn't have that 21 information when I talked to him, but he was right.
22 MR. SCHNEIDER: Yes.
23 MR. WIGGINS: Okay.
24 So, Max, you are believing that this is 25 the primary was the relief that lifted?

2 I will assume that you all know some of 3 the major topical areas, but I'll hit some of the 4 major milestones for you.
5 MR. LANNING: Sure.
6 MR. PRICE: At about 08:29 a.m., this 7 morning, Unit No. 3, Millstone, had a reactor trip and 8 safety injection steam line low pressure.
9 As of right now, because the trip is 10 undetermined, and I understand that there was a 11 previous question with regard to tampering, we have 12 not ruled it out, but we have no reason to believe 13 that tampering was involved. That will be part of our 14 investigation, but once again, we have no reason at 15 this time to believe that tampering was involved with 16 the initiation of the reactor trip and safety 17 injection.
18 Our main safety systems did operate. We 19 currently have - we did go solid in our pressurizer.
20 We have recovered pressurizer level, and pressure is 21 steady, and I have an individual standing in front of 22 the status board, but we have 64 percent in the 23 pressurizer, and we are at roughly 2200 pounds in the 24 primary coolant system. So, we have pressure control 25 and we have pressurizer level restored to scale.

25 1 We have transitioned to our emergency 2 procedure ES1.1, safety injection termination, and we 3 have terminated the safety injection, and we're back 4 on normal charging and let down.
5 We declared our alert Charlie 1 based on 6 our tables, which gives you a direct alert 7 classification based on a main steam safety valve 8 stuck open, but the reason that we classified that is 9 we had a safety valve on the bravo steam generator 10 which had opened below its set point, and right now we 11 don't know if that was the initiator of the safety 12 injection or it came immediately after the safety 13 injection signal, but based on the fact that that 14 steam generator safety valve opened below its set 15 point the shift manager made the classification of 16 alert Charlie 1, and that's the classification that we 17 remain in.
18 We have not yet reached conclusion, but 19 what we are considering here in the EOF is what the 20 termination criteria would be. With the Millstone 21 Unit No. 3 design, we are currently on the atmospheric 22 dump valves, because we have a main steam isolation 23 signal that we have manually injected, so our ability 24 to dump steam to the condenser is limited at this 25 time, and we don't have that capability at this time, NEAL R. 26 1 and we are evaluating our options to restore the 2 ability to dump steam to the condenser.
3 So, right now we are on an atmospheric 4 dump valve that's cycling periodically to remove decay 5 heat.
6 So, we are looking at our termination 7 criteria for the alert, and we believe that we are 8 there right now, but we are also looking at other 9 indications for a notification of unusual event 10 criteria.
11 I don't believe that we see any at this 12 time, but we are still going through the thought 13 process for what we need to do in order to terminate 14 from the event, and as soon as we have reached this 15 conclusion I'll be happy to call you back on this 16 bridge.
17 We do have people at the state armory, 18 last I checked about 15 minutes ago no members from 19 the state had arrived yet, only the Dominion staff was 20 there, and we have a number of members of our 21 corporate EOF staff who have already responded to the 22 corporate EOFs, and we have been in conversations with 23 them.

3 We did put some water from our charging 4 system from a relief valve on a recirc line into the 5 aux building, the last report that I had was that 6 water was clean.
7 MR. HOLIAN: Allen, Brian Owens, have you 8 been operating with any fuel leakers?
9 MR. PRICE: No, sir.
10 MR. HOLIAN: Okay.
11 MR. RYAN: This is John Ryan. We are 12 presently on normal charging and let down.

PRICE: The maximum primary pressure 18 that I believe we have seen has been on the order of 19 2330 pounds, that is below the set point of our power 20 operator relief valves and it's also below the set 21 point of the safety valves. Having said that, we have 22 gotten some information from the Control Room that 23 indicated that they think they had a leakage from a 24 safety valve, and we do have, when last I looked, 25 about 74 or 75 percent level in our pressurizer relief NEAL R. 3 MR. WIGGINS: Allen, this is Jim, just to 4 follow up, do you think the PRT is intact still? They 5 do have a rupture disk, don't they?
6 MR. PRICE: It has a rupture disk, and I do 7 not believe that rupture disk has been challenged as 8 of yet, that is correct.
9 MR. WIGGINS: Okay.
10 MR. PARTICIPANT: This is the Manager of 11 Communications, at the present time we have about 82 =
12 percent in the PRT, pressurizer relief tank, and it 13 shows that it is intact at this time.

15 MR. HOLIAN: John, would you say that 16 again, I could not hear you, the PRT is in tact?
17 MR. RYAN: That is correct, the PRT level 18 is increasing slowly, rising slowly, and it is at 82 19 percent, and the pressure is 64, almost 65 pounds, 20 PSIA.

22 other leakage by itself or otherwise?

8 MR. COLLINS: Jim, this is Sam, for my 9 purposes I'd like to clarify the statement we heard 10 about primary lift versus a primary leak. It sounds 11 like they did not have a lift but there is leakage by 12 one of the valves.

15 MR. PRICE: From the data that I have, Sam, 16 we should not - we did not meet the set point, the set 17 point for a safety valve, but it would appear, once 18 again, I don't have all that data in front of me, that 19 we do have indications from, I believe, a tailpipe, 20 the RTD, that we had indications of a safety valve 21 lift.
22 MR. COLLINS: Okay, and - okay, so any 23 leakage that you would have now would be a result of 24 the event, you didn't have that leakage before, or I 25 presume you would have closed the isolation valve.

You know, 7 there is a rising level in the PRT, something that 8 goes in there is leaking, if they did go solid in the 9 pressurizer I have a sense that these safety valves 10 might not like water very much, they prefer steam, 11 and, you know, what's the way out, Allen? Where are 12 you going to take the plant?
13 MR. PRICE: Well, clearly, our first 14 challenge was to take care of the solid pressurizer 15 conditions. We have done that, and to make sure that 16 we have a stable relationship between the primary and 17 the secondary. I believe we have that.
18 So, we've worked our way through the 19 pressurizer - through the safety injection termination 20 criteria, the communications between the primary and __
21 the secondary, we've got that stable, and now what I 22 have to do is pretty quickly get off this phone and 23 converse with the Control Room and the TSC to 24 understand exactly the answer to that question.

17 The plant, at this present time, is sub-18 cooled by 88 degrees, RCS pressure is 2260, the 19 maximum core exit thermocouples are 555 degrees, 20 reactor vessel level is 100 percent. The RCS - I'm 21 sorry, the pressurizer level is 57 percent. At the 22 present time, we are cooling down the PRT pressure 23 relief tank, using primary grade water, and thatIs why 24 you are seeing, if you are watching the ERDS system 25 you are seeing PRT level and pressure increasing, and NEAL R. 2 The secondary side, as far as the 3 secondary side goes, the steam generator pressure, 4 just a minute that's coming up right now, are 5 approximately 1080 pounds on all four steam 6 generators. We are feeding the steam generators on 7 aux feed, maintaining steam generator level, and 8 radiation levels have not changed since the initial 9 event. They did not change during the initial event.
10 MR. LANNING: John, would you clarify what 11 you mean by that, is this based on a secondary sample 12 or what?
13 MR. RYAN: No, this is based on the 14 radiation monitoring system in the Control Room, rad 15 monitors.
16 MR. LANNING: Have you confirmed you have 17 no primary to secondary leaks?
18 MR. RYAN: At the present time, we have 19 sampled alpha, bravo and Charlie steam generators, and 20 there is no rad levels detected in there, and we have 21 determined that there is no primary or secondary 22 leaks. We are getting a sample on the delta at this 23 time.
24 MR. LANNING: D is in progress?

RYAN: At the present time, the PRT, 7 which is the pressure release tank, is - the level is 8 fluctuating because we are pumping it up and cooling 9 it down with primary grade water, so the detection on 10 that is really not available to me. I can get you 11 tailpipe temperatures on the relief valves and I'll 12 get them while we continue to talk.
13 MR. LANNING: Okay, appreciate that.
14 I understand, are you still in normal 15 charging and let down?
16 MR. RYAN: Yes, we are still doing normal 17 charging and let down.
18 We are still in ES 1.1, which is SI 19 termination criteria, and we are doing a brief right 20 now to enter OP3208, which is RCS cool down.
