Semiannual Performance Data Report for January 1 to June 30, 2005. The data has been analyzed. No program weaknesses were identified during this reporting period.

SUMMARY

During the reporting period January 1 to June 30, 2005, the number of random tests performed was equivalent to the number required to test at an annual rate equal to at least 50 percent of the unescorted access population. The rate of positive results from random testing for this reporting period January 1 to June 30, 2005, was 0.00 percent.

POSITIVE RESULTS Twelve chemical tests yielded a positive result during this reporting period. Calculated from the positives recorded from January 1990 until June 30, 2005, marijuana is the drug of preference, accounting for 61 percent of all the positive results; cocaine is second, yielding 17 percent of all the positive results.

MANAGEMENT ACTIONS Pre-Access Testing There were twelve pre-access testing positive results during this reporting period.

Access to Energy Northwest property was denied for one long-term contractor, nine short-term contractors and two licensee employee applicants for one year. The Medical Review Officer confirmed all twelve pre-access tests positive. Management was notified and the individuals' files were annotated regarding the positive test result and management's actions.

Access to Energy Northwest property was denied for one long-term contractor, one short-term contractor and one licensee employee applicant for three years due to refusing to test. Management was notified and the individuals' files were annotated regarding the refusal to test, access denial and management's actions.

Random Testing There were no positive random testing results during this reporting period.

Follow-Up Testing There were no positive follow-up testing results during this reporting period.

Presently, there is one Energy Northwest employee participating in the follow-up program.

For-Cause/ Observable Behavior Testing There was one for-cause test administered due to observable behavior for a licensee employee during this reporting period. The for-cause test for observable behavior tested negative. The for-cause/observable behavior tests were administered after the following incident:

An Energy Northwest employee failed to re-zero during a procedural step when a backwash sequence was stopped and re-initiated.

For-Cause/Post Accident Testing There were twelve for-cause/post-accident tests administered during this reporting period. All twelve of the for-cause/post accident tests were negative.

The for-cause/post-accident tests were administered after the following incidents:

  • A short-term contractor, operating a jib crane, dropped a new fuel bundle 1/4 to 1/2 inches while in the new fuel inspection stand.
  • A short-term contractor observed another short-term contractor, operating a jib crane, drop a new fuel bundle 1/4 to 1/2 inches while in the new fuel inspection stand.
  • An Energy Northwest employee observed a short-term contractor, operating a jib crane; drop a new fuel bundle 1 to 1/2 inches while in the new fuel inspection stand.
  • An Energy Northwest employee, driving a flat bed truck, struck the roll-up door on the south wall of the Rad/Waste Building.
  • A short-term contractor employee observed an Energy Northwest employee, driving a flat bed truck; strike the roll top door on the south wall of the Rad/Waste Building.

. An Energy Northwest employee, driving a security vehicle, struck an Electrical Security manhole cover causing the cover to move approximately 8 inches.

  • A long-term contractor employee, operating an overhead crane, struck and damaged scaffolding that was recently installed to support the 2005 refueling outage.
  • An Energy Northwest employee was in the process of cleaning a security weapon and while clearing it discharged a round into the floor of a bullet resistant enclosure.
  • An Energy Northwest employee, operating a CAT telehandler, collided with a light pole while moving a skip.
  • An Energy Northwest employee, operating a forklift, hit the west turbine building door with the forklift mast while moving a laundry bin.
  • An Energy Northwest employee, operating a security vehicle, drove into a sign near the security checkpoint.
  • A short-term contractor employee was injured in a fall in the main condenser and had to be transported off-site.

There was one personal injury involving a short-term contractor falling while working inside the main condenser. The short-term contractor was transported to the local hospital emergency department where he was treated and released to return to work on light duty.

Return to Duty Testing There were no return-to-duty tests administered during this reporting period.

Refusal to Test There were three refusals to test during the pre-access testing process. All three individuals had initially provided samples that did not meet temperature requirements. There was one licensee employee applicant, one long-term contractor employee applicant, and one short-term contractor employee applicant who refused to remain in the collection facility until an acceptable specimen could be provided under observed conditions. Access to Energy Northwest property was denied these individuals for three years. Management was notified and the individual's files were annotated regarding the refusal to test, termination for cause and management's actions.

Other Two other tests were administered in the other category. Other testing was conducted as a result of one long-term contractor and one short-term contractor being granted unescorted access but not subject to random selection. The individual's names were not entered into the random selection pool in time for the two individuals to be subject to random selection. Upon discovery, the individual's names were immediately entered into the random test selection pool, and they were chemically tested under the category other, with negative results. This issue was entered into our corrective action process.

ADMINISTRATIVE ACTIONS There were no administrative actions taken during this reporting period.

EVENTS REPORTED TO THE NRC During this reporting period, there were no Fitness-For-Duty events that were reportable in accordance with 10 CFR 26.73.

PROGRAM WEAKNESSES:

No program weaknesses were identified during this reporting period.

PROGRAM CHANGES:

During this reporting period there were no Fitness-For-Duty program changes.
