April 15, 2004 Message from Outage Director Our priorities remain the safe operation of U2 and to safely commence scheduled work activities on Uni 1 and Common Systems using the Unit I R28 Recovery and Restart Checklist.

Our primary focus is to increase RCS Inventory and exit the Safety Assessment "Yellow" conditions on Core Cooling and Inventory. To accomplish this task we need to remove the Reactor Vessel Head, unlatch Control Rods, remove the Reactor Upper Internals, and fill the cavity to refueling height. Prior to removing the Reactor Head we will need to utilize the Containment Polar Crane to move the 1B RCP frpom its location on the pump flange to the motor stand. To make this heavy load movement it is necessary to follow a path over a portion of the Reactor Vessel. A designated safe load path is necessary so the crane operator can minimize the potential for heavy loads that could damage Safe Shutdown Equipment and irradiated fuel in the Reactor Vessel. This requires the Reactor Vessel Head to be in place as added protection for the fuel when the Reactor Vessel Is in the designated load path.

This morning we are in a holding pattern until the Containment Pwolar Crane can be declared ready for use. The Containment Polar Crane operators have reported some anomalies that need to be resolved prior to using it for this important activity. Our Maintenance Department is presently working through a troubleshooting plan with help from a vendor representative. The vender representative arrived on site yesterday afternoon and has made some recommendations to aid in the preparation of the troubleshooting work plan. Work has continued through the night with the vender representative scheduled to return to the site on the day shift to review the results.

Once the Containment Polar Crane is available for use and the Safe Load Path 50.59 Screening is reviewed by the Plant Operations Review Committee then the 1 B RCP Motor lift will be authorized to proceed.

Safety Snippet Avoid a fatal scene; Keep your job site clean A building contractor died when he was trapped in the burning basement of a house under construction.

Another worker was using an oxyacetylene cutting torch to cut I-beams over the basement of the house.

Stray sparks set fire to some sawdust and scrap wood used as packing material. The worker escaped and sounded the alarm, however the contractor jumped into the basement, in a fatal attempt to put the fire out.

CAPo51858, Wrong Cap Removed An operator removed the cap from the incorrect valve when hanging a tag series, In part because the cap had a wrench hanging from it, which was thought to be left by chemistry for the tag series. In reality, this was the location that chemistry leaves their wrench for installation and removal of the sample container. Proper challenging of Information would have prevented this event.
