- Unit 2 May 2000 Dec 2002 o Completed three operating cycles in Unit 1 since S/G replacement e Currently in second operating cycle for Unit 2 since S/Greplacement 2
1IREI 2 Planned SiG Work Scope a Normal S/G Management Program Scope
- Sludge lancing all four steam generators
- Post-lancing Foreign Object Search and Retrieval (FOSAR) all four steam generators
- No primary tube inspections (scheduled for 1 RE1 3)
- S/G 1 D feedring end cap removal, inspection, and FOSAR 3
SiG inspection Results m Small quantities of stainless steel Flexitallic spiral wound gasket material found in S/G's 1A, 1B, and 1C v Numerous feedwater heater (FWH) stabilizing cable wire fragments found in S/G 1D 4
SIG's IA, IB, and IC Results o Pieces of Flexitallic gasket material were found (photos 1-5) e Several pieces were located in peripheral locations in the tube bundle a All identified gasket pieces were successfully retrieved, except for one piece which crumbled when grabbed 5
- Post-retrieval video examination of the affected tubes did not identify any tube wear o Additional inspections The end caps of the feedrings for S/G's 1A, 1 B, and 1 C were removed to facilitate inspection and FOSAR A second video inspection of the tube bundle periphery is planned 6
Cause, Extent, Corrective Action
- Specific source of gasket material not identified
- Gasket damage occurs during installation and degradation soon after return to service
- Introduction of material is not believed to be an ongoing, age-related process
- Non-contained gaskets will be replaced with CGI-style when reworked
- Potential sources scheduled for 1 RE1 2 reworked with CGI-style gaskets
- Processes will be changed for future work 7
SIG ID Background S During 1 RE1 1 shutdown, feedwater (FW) regulating valve 1 D did not fully close o FWH tube stabilizing cable was found wrapped around the valve cage (photo 6) a Some pieces of cable missing due to closing the valve
- A single tube plug was discovered missing from FWH 1 1A outlet side o S/G 1 D only S/G affected 8

& Plug was recovered a Wire fragments were recovered from S/G 1 D feedring spray cans and sludge collector (photos 7-8)
- Potential fragments which may have entered S/G 1 D were determined to be bounded by an existing wear analysis 9
SIG ID Work Scope for IREI2 o Perform previously scheduled sludge lancing e Perform internal bundle inspections and FOSAR a Remove feedring end cap, inspect and clean 10
SIG ID Results f Pre-lancing video inspection revealed multiple wire fragments (photo 9)
- Repeated sludge lancing effectively moved most material to the annulus to facilitate removal (photo 10)

STP Steam Generator 1D NDE Inspection Plan I am attaching the subject tubesheet plans. In words, the scope is: -20% of entire tube population, as planned. The "5 tubes deep' plan has been modified to focus on the critical high velocity areas of interest as follows: +Point 1161 tubes 100% tubes with gap velocities 8.5 fps (at least two tubes deep) 20% tubes with gap velocities 7. 8.5 fps buffer
