- 1. The condition assessment and Issue summary are In the attached document. In summary, the U1 C29 COLR was Issued before the start of refueling operations because of the delay In the outage schedule. The cycle 29 COLR was In the control room for 3.5 days before this CAP Identified the error, and the old COLR was reissued 1.5 days after that. There is no safety significance as a result of this error, because of the depleted reactivity condition of the core, the high boron concentration In the RCS, and the fact that there was no fuel motion during this time.
- 2. The recommended solution is to create an outage activity that links the Issuing of the COLR with specific outage activities, such as refueling, rather than depending on timing.

While verifying required Mode 6 (Refueling) boron concentration (TS 3.9.1), it was discovered that the COLR for Unit I had already been changed to reflect core 29, however due to the outage delays core 28 Is still installed and refueling operations have not yet commenced.
Current Unit I RCS boron Is 2888 ppm, required boron concentration for Mode 6 per COLR for core 28 was 2400 ppm (the required concentration per core 29Is 2200ppm).
