S/G Nozzle Dam Installation Dose Exceeded Estimate 4/11/2004 11:52:45 PM - SHANNON, DAN:
During the installation of the S/G nozzle dams for the U1R28 refueling outage, the dose received (4375 mrem) exceeded the estimate from the pre-job ALARA review.4244e~ffrem).

There were several contributing factors which caused the dose for this job to exceed the estimate. These factors include but are not limited to: 1. Problems with use of bubble hood respirators, 2. Difficulties with Installation of the nozzle dams which caused extra jumps and more time spent on the platform than that expected (based on unit 2). 3. Removal and reinstallation of A S/G Holleg nozzle dam due to not having an RCS vent path, 4. Delays caused by the need for additional personnel to perform jumps. 5. Lack of a proper jump platform for the A S/G jumps.

Engineering programs lead determine lessons learned with input from Scientech and the i F group and implement these lessons prior to nozzle dam removal for unit 1 and the next refueling outage.
