On 5/1 7/2004 at approximately 2345, Power Selector Switch D52A was manipulated by a 1Mechanic Electrician (ME)-during the performance of breaker maintenance.
While performing PM breaker maintenance on H52-20 under W0ft0301793, a ME manipulated Power Selector Switch D52A, in an attempt to energize a breaker test stand. The manipulation of DS2A resulted in momentarily de-energizing power panel D52 which provides DC control power to 13.8kV equipment.
The Control Room received ai annunciator alarm indicating a lockout actuation associated with the 1 X-04 transformer. Operations paged Maintenance to begin determining the potential cause of the annunciator.
In progress testing on the Train B undervoltage relay testing was place on hold pending investigation. Maintenance craft working on troubleshooting D-08 were questioned and maintenance craft working on the H52-20 breaker in the 13.8kV were questioned by Operations.
Maintenance Supervision determined the cause of the Control Room indication to be a result of the switch manipulation.
Maintenance Supervision notified Operations.
A Human Performance Event Investigation was initiated by Maintenance.
