SUMMARY
NRC Performance Summary
- Mitigating Systems White Performance Indicator Safety System Unavailability - Emergency AC Power This Performance Indicator (PI) involved various operability concerns associated with the emergency diesel generators. The PI crossed the threshold from Green to White in the fourth quarter 2001 for both units and remained through the first quarter 2004 for Unit 2, and through the third quarter 2004 for Unit 1
- Mitigating System White Finding Alternative Shutdown Capability and Response Procedures Not Adequate to Ensure Safe Shutdown of Unit 1 and Unit 2 Fire response procedures were not effective in ensuring safe shutdown for a fire in Emergency Switchgear and Relay Room Numbers 1 or 2, of Surry Power Station Units 1 and 2 respectively. Specifically, these procedures may not have precluded an extended loss of reactor coolant pump (RCP) seal injection flow, which could result in an RCP seal loss of coolant accident.
This White inspection finding was identified in the first quarter of 2004

NRC Performance Summary Safety System Unavailability - Emergency AC Power White PI Supplemental Inspection Completed Problem identification, root cause, extent of condition evaluations, and corrective actions were adequate White PI returned to Green in the second quarter 2004 for Unit 2 and the fourth quarter 2004 for Unit 1 EDG 2 start failure occurred on November 7, 2004. This start failure was not evaluated during the 95002 supplemental inspection relative to actions taken for the White PI because licensees root cause evaluation for this issue was still in progress at the completion of the inspection. The NRCs review and assessment of this EDG problem will be documented in a subsequent inspection

NRC Performance Summary Fire Response Procedures Not Adequate White Finding Supplemental Inspection Completed Problem identification, root cause evaluations, and corrective actions were acceptable Initial extent of condition reviews lacked thoroughness and the common cause review did not possess the attributes of an extent of condition review.
NRC observations during the 95002 supplemental inspection resulted in the licensee performing additional and more comprehensive extent of condition reviews. The additional extent of condition reviews, combined with the efforts of the common cause review were determined to be appropriately focused based on the NRCs independent extent of condition review Finding / Violation closed in Inspection Report 05000280,281/2005011
