Note:

Global Nuclear Fuels (GNF) did issue one notice to site on May 14, 2004 - GNF Notification 2003-05, dated May 14"t, 2004. This notification documents correspondence to the Perry site in 2003 discussing an error in the SAFERIGESTR model. This issue was included in the 2003 NRC Annual Letter PY-CEI/NRR-2774L, dated March 1, 2004.

Attachment PY-CEI/NRR-2866L Page 4 of 5 Occupational Radiation Exposure Summary Report Attached is the 2004 Annual Occupational Exposure Summary Report for the Perry Nuclear Power Plant as required by Technical Specification 5.6.1.

The Radiation Work Permit (RWP) under which an individual worked defines the Work Function. An individual may be counted in more than one category.

The Job Function and the Station/Utility/Contract Workers and Others are determined by record of individual signing onto an RWP.

The Number of Personnel >100 mrem are determined by the number of individuals with greater than 100 mrem in the corresponding Work and Job Function.

Both the Number of Personnel >100 mrem and the Total Man-rem are determined using Direct Reading Dosimeters (DRD).

No work activity for 2004 was categorized as Special Maintenance or In-Service Inspection.
