SURRY POWER STATION: NATIONAL EMISSION STANDARDS FOR HAZARDOUS AIR POLLUTANTS (NESHAP) SUBPART M-NATIONAL EMISSION STANDARD FOR ASBESTOS: WASTE SHIPMENT RECORD NOTIFICATION
Dear Ms. Chambers:
Pursuant to 40 CYR 61.150 (d)(4), this serves as the official notification that Dominion's Surry Power Station has not received a copy of a waste shipment record (WSR), signed by the owner/operator of the designated waste disposal site, within 45 days of the date the waste was accepted by the initial transporter.
On September 29, 2004, radiologically contaminated asbestos waste (RCAW) was shipped from Surry Power Station (Shipment Number G2004-5) to Duratek, Inc. (Duratek). The RCAW was received by Duratek on September 30, 2004 (see attached documentation). Once Duratek processes the RCAW, the waste will be shipped to Barnwell Waste Management Facility or Envirocare of Utah for final burial (see Lines 3 and 8 of the WSR).
On November 2, 2004, the 34e day after shipment, Surry Power Station inquired to Duratek about shipment G2004-5 and was informed by Duratek in a telephone conversation that the waste shipment will be processed after the 35dday and will not be buried until after November 13, 2004, the required 45" reporting date. The waste shipment is still located at Duratek in Oak Ridge, Tennessee.
Once Surry Power Station receives the waste shipment record, signed by the waste disposal site owner or operator, Dominion will notify the EPA that the outstanding WSR has been received and closed.
