Single contractor employee tested positive for both Marijuana and Cocaine.

Single contractor employee provided a pre-access fitness for duty specimen that indicated temperature out of range - no temperature.

Individual intentionally left processing facility when asked to provide a second pre-access fitness for duty specimen.

Medical Review Officer was contacted.

Medical Review Officer determination - "Refusal to Test."

25 Administrative Tests While performing a work review on June 15, 2004 it was discovered that a group of 25 individuals were not part of the fitness for duty random testing pool. There was no malevolent intent that led to this situation. The issue was thoroughly investigated and all individuals involved were identified and completed an administrative fitness for duty test without any disqualifying outcome.

Site Access Services and Nuclear Information Services made all the necessary changes to correct the situation. All accounts associated with this group were modified and validated to ensure these individuals were included in the random testing pool.
