Program Performance Data Report for January 1, 2004 through June 30, 2004. This report is submitted pursuant to 10 CFR 26.71 (d). includes summaries of management sanctions imposed, actions completed to improve the program, events reported to the NRC, and our random testing rate. Attachment 2 contains the standard FFD data forms for Surry Power Station, North Anna Power Station, lnnsbrook Technical Center (Corporate), Millstone Power Station, Westinghouse Electric Company and Institute of Nuclear Power Operations (INPO).

SUMMARY
OF MANAGEMENT ACTIONS COMPLETED TO IMPROVE THE FFD PROGRAM JANUARY 1 THROUGH JUNE 30,2004 No significant changes were made to the Fitness for Duty Program during this reporting period.
SUMMARY
OF EVENTS REPORTED TO THE NRC BY DOMINION JANUARY 1 THROUGH JUNE 30,2004 On February 2,2004, a licensed operator was randomly tested at North Anna Power Station. Although the tested level was below the 10CFR26 cut-off level for alcohol, the individual was determined to be unfit for scheduled work activities. In accordance with 1 OCFR26.73(a) and (b), a 24-hour report was made to the NRC on February 2,2004.
On June 14,2004, a Dominion supervisor was pre-access tested at Surry Power Station upon his return to work and was confirmed positive for alcohol. The individuals unescorted access had been terminated on March 1,2004, and his unescorted access authorization placed under review as a result of a Fitness for Duty self-reporting notification. The individual was terminated from employment. In accordance with 10CFR26.73(a) and (b), a 24-hour report was made to the NRC on June 14,2004.
