Fire System Pressure Perturbation While restoring from a modification on a portion of the Fire System, an eight-inch valve was opened to refill a section of system piping. The system perturbation caused both Fire Pumps to auto start. The resulting pressure transient caused significant packing leakage on the eight-inch valve, and the lifting of various deluge system relief valves. (ER 2003-2264)
Neither the Mod package step text nor the switching/tagging restoration instructions provided guidance on ho~w to minimize/eliminate this pressure transient while refilling the section of voided piping.
What's the REAL message?
We work in a very complex and integrated plant environment. When preparing documents such as plant modifications, work orders, and switching and tagging instructions, the integrated effects of each action taken must be considered. "What if...?" must be asked. Appropriate barriers / instructions must be included in instructions.
QUESTIONING ATTITUDE isn't limited to the person lifting the lead, turning the wrench, or opening the valve. A QUESTIONING ATTITUDE must be applied in evervthing we do. A great way to increase the number of questions asked when performing any task (including the preparation of documents) is to ask for a PEER CHECK.
