NRC EXAM HIGH PERCENTAGE MISSED QUESTION ANALYSIS Question #6 Five students missed this question. All five individuals selected answer C. The Operator performing Auxiliary Building Actions will locally control Centrifugal Charging Pump Discharge Flow Control Valve, BG FCV-121.

Answer C is incorrect. In OFN RP-017, Pressurizer level is maintained by throttling the BIT outlet valve, and is performed by the person assigned to the Turbine Building attachment. Answer C would only be correct for a situation in which OFN RP-013, CONTROL ROOM NOT HABITABLE was being used.

Question #8 The question asks what action is needed first from OFN EG-004, CCW SYSTEM MALFUNCTION while attempting to identify and isolate the leak?

WCGS normally operates with only one CCW train in service; Step 1 RNO column will start a CCW pump in the idle train. Then, Step 3 RNO will place Emergency Make-up in service. Attempts to identify leak location are continued, and if the leak has not been located/identified, the operator is directed to shift the Service Loop to the alternate train (if the leak is in the loop supplying the Service Loop).

The question requires the examinee to determine which action(s) are to be performed first. Answer A (key correct answer) says Align ESW makeup to the CCW surge tank, then direct personnel to locate the leak. These actions would be taken at Step 3. RNO, and then Step 4 RNO. Answer B says Start a CCW pump in the alternate train, then shift service loop to that train. These actions would be taken at Step 1 RNO, and Step 5 RNO.

Response: Based on review of the stem and use of the word first, it is determined that answer B is correct and since answer A does not start a pump it is incorrect. Answer Key, exam question and candidates grades were all changed to reflect B as the correct answer.

The question asks Why does the swap-over from the CST to ESW occur?

Answer A The CST level corresponding to the LSP signal is inadequate to maintain Auxiliary Feedwater Pump suction head. This answer should be considered correct if one assumes that the suction head will not be maintained for long after reaching this setpoint. Answer D, The CST is not seismically qualified and ESW system is seismically qualified is the KEY answer. This is a correct statement, and describes why ESW was chosen as the alternate supply to AFW.

Answer D is the only correct answer for the conditions given.
