They have included near misses and one individual receiving a shock (he was not injured). With the amount of electrical work left to do in this outage, WE MUST KEEP OUR FOCUS ON ELECTRICAL SAFETY. Please take a minute to read the attached yellow memo which discusses these issues and what you can do to keep yourself and your co workers SAFE. Worker Receives a Shock The Event: Condition Report 2004-01235 documents that an employee came in contact with a live terminal while moving a relay during implementation of a design modification. The circuit had previously been tagged out but had been re-energized via a temporary lift. The Supervisor did not appropriately respond when notified of the Temporary Lift and the worker did not-confirm that the circuit was dead before proceeding with the work. Safety was notified and the individual was checked out at the hospital and was not injured. What went right?

  • Work was stopped, Supervision and Safety were notified and the worker was not injured.

What went wrong?

  • The Supervisor did not appropriately respond to a warning in ESOMS advising of C the "Temporary Lift"U A "live-dead-live" check as required by Safety Standard 302 was not performed Several other Condition Reports have been written within the last 24 hours reporting' Electrical Safety vulnerabilities g

What'sthe mesiage? s De-energizing an electrical bus or panel does not necessarily de-energize everything

  • "Live-Dead-Live" Checks are required by the Safety Standard
  • Compliance with Safety Standards is a condition of employment
  • Tagging must be verified to be correct, complete and in place
