INTRODUCTION The reactor operated continuously in June with the following exceptions: 4 shutdowns for scheduled maintenance and refueling; 2 unscheduled shutdowns.

MAINTENANCE ACTIVITIES 6/3/04 Refueled - removed core 04-25, loaded core 04-26.

6/7/04 Refueled - removed core 04-26, loaded core 04-27.

Reinstalled refurbished Secondary Coolant System Distribution Valve S-19.

Replaced cooling water lines to Primary Coolant Pumps 501AIB.

6/14/04 Refueled - removed core 04-27, loaded core 04-28.

6/16/04 Refueled - removed core 04-28, loaded core 04-29.

6/17/04 Shipped eight spent fuel elements to Savannah River Site.

6/21/04 Refueled - removed core 04-29, loaded core 04-30.

Removed the 20 element X fuel storage basket for inspection.

6/28/04 Refueled - removed core 04-30, loaded core 04-3 1.

Placed an epoxy patch on the secondary side of Primary Heat Exchanger 503A.

6/29/04 Loaded new pool de-ionization bed 'F'.

UNSCHEDULED SHUTDOWNS Date No. Type Cause 6/3/04 NA Manual Sample scheduling error Scram On June 3, the reactor was shutdown due to a sample scheduling error which resulted in the incorrect sample being removed on the previous maintenance day. After the shutdown, the correct sample was pulled. The reactor was subsequently returned to 10 MW operation.

6/16/04 NA Manual Inner Airlock Door Positioning Chain Scram On June 16, the reactor was shutdown when it was discovered that the inner airlock door positioning chain had come off it's sprocket due to a loose set screw. Containment integrity was never lost since the outer airlock door remained closed and the seal inflated.

The reactor was secured, and the inner airlock door set screw was replaced with a locking style set screw. The reactor was subsequently returned to 10 MW operation.
