(OP 2311.009) enhanced place-keeping and procedure controls Target: April 30, 2004 CRDM/ UT of 60 CRDM nozzles UT of the RPV nozzle from 2" above the highest UT scans of CRDM nozzles underway RADCAL (no known defects) point of the root of the J-weld to the blind zone.

Nozzles

• 50 of 60 nozzles inspected

• 46 of 60 analyzed by Entergy (Note 1)

One nozzle confirmed to require repair UT of six FTI repaired UT the weld and at least 1-inch above the weld in 3 FTI repairs inspected CRDM nozzles (Nozzles the nozzle base material. No analysis at this time 3, 6,15,17, 33, & 56)

UTiWS of two W repaired UT of the RPV nozzle from 2" above the highest CRDM nozzles (Nozzles point of the root of the J-weld to the blind zone 54 & 68) and wetted surface exam of weld overlay.

Walkdowns RCS leaks boric acid leaks from pressure-retaining

• 18 of 18 pressurizer/CFT nozzles inspected components as well as other A600 RCS small

• 14 o14 hot leg nozzles inspected bore locations. (OP 2311.009)

• 11 of 13 Cold Leg nozzles inspected No indications identified Note 1: The freespan length below the weld is being reviewed and has been preliminarily confirmed on 40 of 46 analyzed nozzles to meet Entergy's Order Relaxation request of February 11, 2004 (CNRO-2004-0008).
