SUBJECT:

MINUTES OF INTERNAL MEETING OF THE DAVIS-BESSE OVERSIGHT PANEL The implementation of the IMC 0350 process for the Davis-Besse Nuclear Power Station was announced on April 29, 2002. An internal panel meeting was held on January 22, 2004. Attached for your information are the minutes from the internal meeting of the Davis-Besse Oversight Panel.

MEETING MINUTES: Internal IMC 0350 Oversight Panel Meeting Davis-Besse Nuclear Power Station DATE: January 22, 2004 TIME: 1:30 p.m. Central ATTENDEES:

1. Discuss/Approve Todays Agenda The Panel approved the agenda, but modified the order of presentations. THE APPROVED AGENDA REFLECTS THE ORDER LISTED IN THESE MINUTES.
2. Panel Review Restart Checklist Items 4.b, Effectiveness of Corrective Actions in the Organizational Effectiveness and Human Performance Area to Define Remaining Closure Criteria Considering Findings from Followup Management & Human Performance Team Inspection The Panel discussed Restart Checklist Items 4.b, Effectiveness of Corrective Actions in the Organizational Effectiveness and Human Performance Area to Define Remaining Closure Criteria Considering Findings from Followup Management & Human Performance Team Inspection. G. Wright summarized a portion of his inspection regarding safety culture and safety conscious work environment. Geoff's team determined that an improvement in safety culture and safety conscious work environment has occurred on a site-wide basis; however, a recent survey taken in November of 2003 called into question the effectiveness of some of the Corrective Actions that were required by 10 CFR Appendix B Criterion XVI. Geoff's team plans additional inspection next week to understand the cause of decline in certain questions related to safety culture and safety conscious work environment as a result of site-wide surveys taken in March and November 2003. The Panel determined that there is no definitive criteria for closure of Restart Checklist Item 4.b. Rather, the closure will be based on evaluation by the Panel of the integrated results of various performance-based inspections and inspections conducted by the Management & Human Performance Team.
3. Panel Discuss Recommendation for Followup Inspection to Ensure Employee Overtime and Fatigue Issues are Not Safety Concerns for Restart.

The Panel discussed followup inspections to ensure employee overtime and fatigue issues are not safety concerns for restart. G. Wright discussed his teams inspection next week and plans to integrate employee overtime and fatigue issues. The Panel reviewed a recent allegation regarding fatigue and overtime and determined that the issue should be presented to an allegation review board. D. Passehl was assigned action to prepare a package to present to the board. In addition, D. Passehl took action to research the Panels previous restart criteria concerning allegations and research the late-breaking allegation process to reaffirm restart criteria for allegations at next Panel meeting.

4. Other A. Mendiola took action to coordinate NRR and Region III allegation folks regarding review of a letter regarding the root cause assessment of the reactor vessel head at Davis-Besse.
