(a) NUREG-1021, Draft Rev. 9, ES-202, Preparing and Reviewing Operator Licensing Applications, Section D, License Eligibility Guidelines (b) 10 CFR 55.31(a)(4), Application Requirements for Operator's Licenses Attachments (1) and (2) provide the educational history for the subject applicant, as requested by Mr. Joseph D'Antonio, United States Nuclear Regulatory Commission Region I - DRS. These attachments were requested by Mr. D'Antonio based upon discussions with Calvert Cliffs Nuclear Power Plant Training Department personnel prior to the upcoming Initial Operator Licensing Examinations scheduled for March 2004. This information is provided to assist Nuclear Regulatory Commission personnel in determining the applicant's conformance with eligibility requirements specified in References (a) and (b) for an initial operator license. These attachments are only being provided to Mr. J. M. D'Antonio.
