Davis-Besse Integrated Report, Appendix A, B, C, D, & E Good morning, Appendix A Item 3 If the frequency and/or voltage values are worse than the values of other nuclear plants, and are accepted (grand fathered), then I think there should be use limits.
For example, after 5 start and loading cycles, the electric motors experiencing these values will be rebuilt.
Item 6 What about the next outage that is actually 2 weeks or longer, planned or not?
Item 11 This item assumes everybody needs to remain employed.-
Item 20 Shouldn't the Cycle 14 plan actually begin at the start of Cycle 14?
Appendix B NRC Restart Checklist Item S.d change "(including visual inspection of.." to "(including video inspection of.."

What you need here is a very strong emphasis on individual self improvement of technical skills.
I don't see it here.
One way to provide such encouragement would be with tuition assistance to those working and going to school at night to acquire more engineering skills.
Continuous Safety Culture Improvement Item 4 Why, at this late date, would any employee not have completed SCWE training?
Completion of this item should be PRIOR to restart.
Procedure Improvement Item 2 Check that the FENOC standards are proper first.
Corrective Action Program Improvement Item 2 Perhaps the appropriate level should be based on the cost of repair/modifications, not simply the number of them.
Internal and External Oversight Improvement

Where did they suddenly acquire assessment ability?
Individual Barrier Attributes I-07 Change back to ZERO as mentioned in the Integrated Report.
One is NOT acceptable for a goal in an organization trying to show it is no longer production dominated.
I-08 See immediately above.
I-15 This number might be too high.
I would think a lower number, say 90% might be acceptable if a severe penalty was assessed upon failure of the follow-up remedial training.
Programs Barrier Attributes P-04 I don't understand what this goal means.
P-05 One operating cycle is too short for this goal.
Management Barrier Attributes M-03 Change goal from "<1.25" to 1.
M-04 Change goal from "<2" to 1.
M-06 Change goal from "Managers-and supervisors are generally effective with a few exceptions" to "Managers and supervisors are effective" M-12 Change from "<100" to "10."
M-13 Change "completed within 3 months of schedule" to "completed on schedule" (Note:
these Management Barriers mostly sound to me like they are production oriented.)
Oversight Barrier Attributes 0-06 Use of Industry peer support acceptable if it doesn't include INPO participation.
Appendix E Item 82 Is this for Boiling Water Reactors or Pressurized Water Reactors?
