Status of ANO-2 RV Head Inspections October 3, 2003
- Tom, As of 6:00 AM Friday, 10/3/03, the status of the ANO-2 RV head inspections is as follows:
Dose Total Dose to date on RVH inspections 18.5 Rem Total Dose to date on Pzr Inspections/lMNSA 2.09 Rem CEDM UT and LF ECT exams 66 of 81 nozzles have data acquired 65 of 66 have been analyzed 65 of 58 have been accepted by Entergy No detectable indications Continue data acquisition CEDM Augmented manual ECT 37 of 75 have data acquired. No indications to date Inspection tooling has been modified to the extent possible. Continue ECT exams, as access allows The outer 32 CEDM nozzles have been scanned with the handheld tool on the upper portion of the blind zone. The lower portion of the blind zone will be hand scanned on day shift.
Due to tight spacing of the alignment cones, the three corner nozzles of the 7 by 7 array of CEDMs (15 total) are not readily accessible. Neither the manual ECT tool or the hand held probe is currently able to get to the blind zones of these nozzles. This includes nozzles 41, 60, 42, 43, 61, 44, 45, 58, 38, 39, 59, and 40. See attached drawing ICI UT Exams 8 of 8 nozzles examined. Two nozzles (84 and 85) have been rescanned several times with incomplete data due to contact 6 of 8 analyzed No indications identified ICI augmented manual PT exams PT exams performed on 8 of 8 nozzles 2 of 6 nozzles had surface indications (Nozzles #83 & #84)

Nozzle being stress relieved, no repair.
ICI nozzle bare metal visual & RVH visual exams (Complete) 8 of 8 have had 360 degree boroscopic exam performed No boric acid detected Also no boron on flange areas Initial visual exam and video taping of top of RV shroud completed Limited boroscope inspections on shroud confirmed no boron Vent line ECT exam (Complete)
No detectable indication Pressurizer Heater Sleeve MNSA inspections (Complete)
No new heater sleeves leaking 6 of 6 installed MNSAs inspected for leakage 2 of 6 had potential leakage 2 of 2 MNSAs were disassembled 1 of 2 was leaking Metal was removed from Counterbore MNSA reinstalled second of 2 did not show any signs of leakage White residue was concluded not to be RCS.
