STP Draft Outline Comments Written Test (NRC Developed)

RO Outline

  • Have too many K/As (6) associated with the pressurizer. See ES-401, Section D.1.d.

Recommend that you deselect 3 of the pressurizer system questions and select the same Category from a different E/APE or system.

Tier 1/Group 1: E/APE 000008 and 000027 Tier 1/Group 2: E/APE 000028 Tier 2/Group 2: System 010 (2 questions) - deselect one question from this group.

Tier 2/Group 2: System 011 SRO Outline

  • For Tier 2/Group 1, deselect System 010 and pick another system.

Recommended changes made. Additional changes also made - see Form ES-401-4 and licensees recommended changes.

Operating Test (STP Developed) Discussed with Mike Defrees on 3/27 (comments in Redline)

Scenarios:

  • Need Forms ES-301-4, 301-5 and 301-6 for the spare scenario. Agreed not necessary to fill out form 301-5 and -6.
  • Scenarios 1, 2, and 3 all have an instrument failure associated with the S/G which effects control of S/G water level. Recommend modifying one of the scenarios to have an instrument failure on a different system. Modify Scenario 2 by removing Event 1 and replacing iwht a SG PORV press instrument failure, causing PORV to open.
  • For Scenario 1
1. appears that there are 2 major transients - tube rupture and steam leak inside the RCB or the steam leak is another component failure. The steam is NOT going to be in the scenario, so comment is not applicable.
2. Critical tasks - why is the second task critical? For Event 3 - if the operator fails to properly control S/G level manually, then the plant would trip on low S/G water level. Should this be a critical task? Change Critical Task 1 to no Rx trip -

properly control S/G water level. Since the steam leak is not going to be in the scenario, will no longer transition to EC31, so the original critical task not valid anymore. Change wording on Critical Task 2 to maintain SG pressure above

??? to prevent transitioning to EC31..

3. Need to verify that all candidates get another scenario, since Scenario 1 is a bank scenario. Refer to Step 10 of ES-301-4.
4. ES-301, D.5.b., requires at least one new or significantly modified scenario.

SROU 3 is only getting a bank scenario (Scenario 1) in the SRO position. He is getting another scenario, but in the BOP position. Does this meet the intent of the step? This would ONLY apply to the 12 man schedule!!!

  • For Scenario 2
1. Critical tasks - Event 1 and 2 - if the operator does not take proper action, the plant would trip on low S/G water level. Should these events be critical tasks?

Replacing Event 1 with SG PORV press instrument failure and for Event 2 have deaerator and not time critical > 5 minutes.

  • For Scenario 3, Event 6 and Control Room JPM S3, verify that this JPM is sufficiently different from Event 6 so as not to have overlap between the operating exam.

Event 6 the diesel ties on to the bus, but loads dont sequence on. JPM S3 dead bus and candidate ties diesel to bus and ECW pump wont start.

  • ES-D-1 needs a time line. Will include times on the ES-D-1 with the draft exam.

JPMS:

  • Recommend that SRO Admin JPM A8 be done after one of the Scenarios. This will provide a more realistic JPM. Classification of event will be based on simulator freeze point.
  • Control Room JPM S6, Establish Supplementary Purge, has K/A listed as 028A4.01 or 103A4.01. Recommend you list a K/A starting with 029, since this JPM is taking credit for Safety Function VIII. Changed to 029A1.03.
  • Control Room JPM C2, Respond to RCB High Radiation, is listed as Safety Function IX, is this correct??? Should it be Safety Function V or VII? Per ES-401-2, W/E16 High Containment Radiation is listed as Safety Function 9.
  • For the Admin JPMs, which ones are bank, new, or modified? Need to know this so, I can evaluate the SROs requirement that no more than 8 of 10 JPMs can be taken from the bank. For the SROs control room and walk through JPMs, the 5 JPMs are all bank.

Added modified or new to the Admin outlines - meets the requirements for new or modified.

Form ES-301-5:

  • For RO2, RO3, and RO4, they meet the minimum requirement of 5 instrument/component evolutions, but recommend adding 1 additional failure. If one of the candidates missed one of the 5 failure, we would have to run the spare scenario.

Will add Event 8, AFW Pump 11 fails to start (remove OOS from initial conditions), to Scenario 1.

Form ES-301-6

  • For RO5 and RO6, in the RO position for Scenario 3, no credit for Event 4 (this is an BOP response, see ES-301-5 and ES-D-1). RO will either manually trip the Rx or being doing actions associated with an automatic trip of the Rx.
  • For I1 and I2, in the BOP position for Scenario 3, no credit for Event 6 (this is an RO response, see ES-301-5 and ES-D-1). BOP will be starting AFW pump in response to Event 6.

Per discussion with Mike, will leave ES-301-6 forms as is.

Schedule (11 Person Schedule)

  • On Tuesday, Crew A, Admin (0900-1100), typo for Crew A composition - should be R2 vs R3. Mike will correct on future schedule.
  • Based on current Schedule, assign E4 to me (Greg Werner). If so desired, I believe this would give me the flexibility to do a late (1730) Thursday afternoon exit. I would have time on Wednesday afternoon and maybe Thursday afternoon to prepare. Will try to do this so NRC can exit on Thursday afternoon.
