On November 10, 2003 at 0810 hours0.00938 days <br />0.225 hours <br />0.00134 weeks <br />3.08205e-4 months <br />, TS 3.3.3.1 was entered when Salem Unit 2 entered Mode 4 from its thirteenth (2R13) refueling outage. TS 3.3.3.1, Action b requires that With one or more radiation monitoring channels inoperable, take the ACTION shown in Table 3.3-6." Action 26 of Table 3.3-6 requires that within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> the preplanned alternate method of monitoring the appropriate parameter(s) be initiated, and (a) the inoperable channel be restored to operable within 7 days, or (b) to prepare and submit a Special Report to the Commission pursuant to Specification 6.9.2 within 14 days following the event outlining the action taken, the cause of the inoperability and the plans and schedule for restoring the system to OPERABLE status.

As stated above, the channel became inoperable when Unit 2 entered Mode 4 and condenser vacuum could not be established. Condenser vacuum could not be established due to delays during the generator shaft and exciter alignment. Condenser vacuum was established on November 18, 2003 and the 2R15 action statement was terminated.
