  • Corrective Action Program (CAP) failures contributed to the Head Degradation
  • FENOC initiated actions to improve the corrective action program 10

  • Reviewed over 400 condition reports
  • Reviewed numerous corrective action related documents
  • Observed & assessed various management oversight corrective action meetings and conducted interviews

  • CATI identified 28 examples of potential violations of NRC requirements
  • Violations appeared to be of very low safety significance
  • Concerns were identified in three corrective action implementation areas 12
