  • Davis-Besse vessel Head Degradation identified in February 2002
  • Corrective Action Program (CAP) failures contributed to the Head Degradation
  • FENOC initiated actions to improve the corrective action program

11 Davis-Besse Oversight Panel - Public Meeting November 2003 CATI INSPECTION ACTIVITIES

  • Reviewed over 400 condition reports
  • Reviewed numerous corrective action related documents
  • Observed & assessed various management oversight corrective action meetings and conducted interviews
  • Reviewed Corrective Action Program implementation audits and assessments

  • CATI identified 28 examples of potential violations of NRC requirements
  • Violations appeared to be of very low safety significance
  • Concerns were identified in three corrective action implementation areas

  • Effectiveness in the identification and cause evaluation of design and equipment issues
  • Effectiveness in the implementation of corrective actions to address engineering issues (adequacy of calculations)

  • Team concluded that the corrective action process has improved, however, significant staff effort and vigilance is needed
  • Significant management oversight is warrented to continue needed improvements
  • Effectiveness reviews need to be completed to accurately assess Corrective Action Program long term viability
