Attached is the Discharge Monitoring Report for the Hope Creek Generating Station for the month of September 2003.
This report is required by and prepared specifically for the Environmental Protection Agency (EPA) and the New Jersey Department of Environmental Protection (NJDEP).
It presents only the observed results of measurements and analyses required to be performed by the above agencies. The choice of the measurement devices and analytical methods are controlled by the EPA and the NJDEP, not by the company, and there are limitations on the accuracy of such measurement devices and analytical techniques even when used and maintained as required. Accordingly, this report is not intended as an assertion that any instrument has measured, or that any reading or analytical result represents the true value with absolute accuracy, nor is it an endorsement of the suitability of any analytical or measurement procedure.

LR-E03-0395 NJPDES DMR EXPLANATION OF CONDITIONS September 2003 The following explanations are included to clarify possible deviation from permit conditions.
General - The columns labeled "No. Ex" on the enclosed DMR tabulate the number of daily discharge values outside the indicated limits.
Data reporting and accuracy reflect the working environment, the design capabilities and reliability of the monitoring instruments and operating equipment.
Deviations from required sampling, analysis monitoring and reporting methods and periodicities are noted on the respective transmittal sheet.
Results reported on the Discharge Monitoring Report forms are consistent with permit limits, data supplied from contract laboratories, the December 1993 revision of the NJDEP DMR Instruction Manual and specific guidance from DEP personnel.
LR-EO3:0395 NJPDES DMR EXPLANATION OF EXCEEDANCES September 2003 The following exceedance is included in the attached report and explained below.
- 1. A description of the discharge, including the time of the discharge, the location of discharge, the volume of the discharge, the concentration of pollutants discharged, and the receiving water of the discharge; On Sept. 11, 2003, the vendor laboratory notified PSEG Nuclear LLC - Hope Creek Chemistry that the sewage treatment plant effluent, DSN 462B, fecal coliform sample result for the sample taken on 9/3/2003 was 620 col/1 00 ml. Hope Creek Station NJPDES permit NJ0025411 has a monthly average limitation of 200 col/100 ml and a weekly average limitation of 400 col/1 00 ml. The sample analysis result exceeded the weekly average limitation.
DSN 462B co-mingles with the cooling tower blowdown flow and ultimately discharges to the Delaware River via DSN 461A. It is unknown what the total volume of the discharge was.
- 2. Steps being taken to determine the cause of the permit noncompliance; When the sewage treatment plant operator learned of the fecal coliform exceeding NJPDES limits, he walked down the sewage treatment plant system. He found the chlorine tablets, used to the treat sewage plant effluent, were discolored with a gray appearance. The operator replaced the chlorine tablets in use with fresh chlorine tablets from an unopened container. The new tablets were bright white with a strong chlorine odor.
- 3. Steps being taken to reduce, remediate, and eliminate the noncomplying discharge and any damage to the environment, and the anticipated time frame to initiate and complete the steps to be taken; After the tablet replacement on 9/11/03, subsequent fecal coliform results were:

The duration of the discharge, including the dates and times of the commencement and, for an unanticipated bypass, the dates and times of the end or anticipated end of the discharge, and if the discharge has not been corrected, the anticipated time when the permittee will correct the situation and return the discharge to compliance; It is unknown when the discharge started but after replacing the chlorine tablets subsequent samples were below permit limits.

The cause of the noncompliance; The cause appears to be the use of degraded chlorine tablets. Additionally, the vendor laboratory performing the analysis did not notify PSEG Nuclear personnel in time to allow system adjustments and additional grab sampling analysis to reduce the weekly average to within the limit.
- 6. Steps being taken to reduce, eliminate, and prevent reoccurrence of the noncomplying discharge; Establish a routine to replace chlorine tablets with fresh tablets on a periodic basis.
Review the lessons learned from this event with the contract sewage treatment plant operators and reinforce the need for a questioning attitude and self-checking. Clarify expectations for timely sample result follow-up to allow proactive operator actions.
Review expectations with the vendor laboratory and if necessary create expectations to immediately notify PSEG Nuclear of exceeded permit limits. Provide revised PSEG Nuclear Chain of Custody forms with contact information and PSEG Nuclear NJPDES permit limits with special instructions for immediate notification.
