Dear Ms. Beckman:

This confirms the verbal authorization that was provided to Beckman and Associates, Inc.

(BAA), effective July 11, 2003, to begin work under the subject task order, with a temporary ceiling of $20,000.00.

In accordance with Section G.5, Task Order Procedures, of the subject contract, this letter definitizes the subject task order. The effort shall be performed in accordance with the enclosed Statement of Work.

Task Order No. 015 shall be in effect from July 11, 2003, through October 1, 2003, with a cost ceiling of $38,306.35. The amount of $37,083.53 represents the estimated reimbursable costs, and the amount of $1,222.82 represents the fixed fee.

Amt.: $36,000.00 Incremental funds in the amount of $36,000.00 are being obligated under this task order. The obligated amount shall, at no time, exceed the task order ceiling. When and if the amount(s) paid and payable to the Contractor hereunder shall equal the obligated amount, the Contractor shall not be obligated to continue performance of the work unless and until the Contracting Officer shall increase the amount obligated with respect to this task order. Any work undertaken by the Contractor in excess of the obligated amount specified above is done so at the Contractor's sole risk.

The following individuals are considered to be essential to the successful performance for work hereunder: The Contractor agrees that such personnel shalnot be removed from the effort under the task order without compliance with Contract Clause H.4, Key Personnel.

The issuance of this task order does not amend any terms or conditions of the subject contract.

CONTRACT NRC-03-03-037 STATEMENT OF WORK Task Order No. 015 TITLE: Davis Besse Program Review Inspection INSPECTION REPORT NUMBER: 50-346/03-012 B&R NUMBER: 320-15-103-142 JOB CODE: J-3020 NRC PROJECT OFFICER: Donald Norkin, NRR, (301) 415-2954 TEAM LEADER: Geoff Wright, Region III, (630) 829-9602 PERIOD OF PERFORMANCE: 07/11/03- 10/01/03 BACKGROUND Region IlIl is reviewing licensee actions preparatory to restart of the Davis Besse nuclear plant, near Toledo, OH.

OBJECTIVE The objective of this task order is to obtain expert technical assistance to support NRC inspection teams in evaluating preparations for the restart and operation of the Davis Besse nuclear plant.

Activities include evaluating the licensee's:

(1) Restart Readiness Review meeting; (2) Safety Conscious Work Environment Review Team meeting; (3) long term safety culture assessment tool; and (4) corrective actions to the external safety culture report.

WORK REQUIREMENTS AND SCHEDULE It shall be the responsibility of the contractor to assign qualified technical staff, employees, and subcontractors, who have the required educational background, experience, or combination thereof, to meet both the technical and regulatory objectives of the work specified in this Statement of Work (SOW). The NRC will rely on representation made by the contractor concerning the qualifications of the personnel proposed for assignment to this task order including assurance that all information contained in the technical and cost proposals, including resumes and conflict of interest disclosures, is accurate and truthful.

The Team Leader may issue technical direction from time to time during the duration of this task order. Technical direction must be within the general Statement of Work stated in this task order, Attachment

and shall not constitute new assignments of work or changes of such nature as to justify an adjustment in cost or period of performance. The contractor shall refer to the basic contract for further information and guidance on any technical directions issued under this task order.

Any modifications to the scope of work, cost, or period of performance of this task order must be issued by the Contracting Officer and will be coordinated with the Project Officer. Specific tasks under this task order are:

Contractors will observe the licensee's Restart Readiness Review meeting, which is currently scheduled on site on, or about, July 14-16, 2003. Subsequent activities will be reviewed on a schedule commensurate with licensee readiness and availability of NRC and contractors.

REPORT REQUIREMENTS A feeder to the final inspection report shall be provided to the Team Leader in an electronic format acceptable to the Team Leader. A hard copy shall be provided to the Project Officer. The contractor shall not undertake any further efforts toward report finalization, such as management review of the feeder report.

TRAVEL (for estimating purposes only) for each individual:

Two trips to the plant site Five days travel The contractor shall coordinate all travel arrangements in advance with the NRC Team Leader.

NRC FURNISHED MATERIAL Documents required to prepare for the inspection will be provided by the NRC Team Leader.

OTHER APPLICABLE INFORMATION The work specified in this SOW is 100% licensee fee recoverable. The contractor shall provide fee recovery information in the monthly progress reports in accordance with the requirements of the basic contract.
