INTRODUCTION The reactor operated continuously in August with the following exceptions: 6 shutdowns for scheduled maintenance and refueling; 1 unscheduled shutdown.
MAINTENANCE ACTIVITIES 8/4/03 Refueled - removed core 03-35, loaded core 03-36.
Relocated Room 202 evacuation switch during asbestos abatement and remodeling.
Removed Room 202 Fan Failure Alarm Panel in accordance with Modification Record 88-7; Addendum 2.
8/7/03 Refueled - removed core 03-36, loaded core 03-37.
8/11/03 Refueled - removed core 03-37, loaded core 03-38.
Replaced temperature element 980B transmitter.
8/18/03 Refueled - removed core 03-38, loaded core 03-39.

Type Cause 8/7/03 1167 Scram Rod Position Indication Failure On August 7, a Reactor Scram was manually initiated by the operator when a fault in the Rod Position Indication System resulted in fluctuating position indication for Control Rod 'D'. Troubleshooting revealed a broken ground wire to the position indication amphenol. The broken wire was fixed and the integrity of the remaining control rod position indication amphenols were verified sat. The Rod Position Indication System was tested sat. The reactor was subsequently refueled and returned to operation.
