Enter the level of difficulty (LOD) of each question using a 1 - 5 (easy - difficult) rating scale (questions in the 2 - 4 range are acceptable).
Check the a propriate box if a sychometric flaw is identified:

U / DO NOT COUNT There were several items that were UNSAT but we did not count them as UNSAT toward the total number of UNSAT for the 20% report count. The reason is as follows:

In some cases the license call and asked to change WA because they could not match. The NRC told Entergy to keep the same WA. This resulted in an UNSAT and WA swap after the exam was reviewed.
ES 501 section E.3.a, states in part Note: NRC-validated questions, JPMs,
, and scenario events that required replacement or substantial modification will not be counted unless the reason for the current unacceptable flaw was caused by the facility licensee since the time the test item was previously approved by the NRC. (For example, the questions reference changed but the question was not revised accordingly.)
In all cased these questions were corrected before the exam was given.
