INTRODUCTION The reactor operated continuously in April with the following exceptions: 4 shutdowns for scheduled maintenance and refueling; 3 unscheduled shutdown. Received notification from the NRC that one new Senior Reactor Operator license had been issued.
MAINTENANCE ACTIVITIES 4/6/03 Refueled - removed core 03-13, loaded core 03-14.
Replace PRM channel 6 detector and cabling.
4M103 Refueled - removed core 03-14, loaded core 03-15.
4/14/03 Refueled - removed core 03-15, loaded core 03-16.
4/21/03 Refueled - removed core 03-16, loaded core 03-17.
4/23/03 Refueled - removed core 03-17, loaded core 03-18.
Replaced card cage for the Iodine Channel in the Eberline Ping A Stack Monitor.

Type Cause 4/6/03 1158 Scram PRM Channel 6 detector cabling shorted On April 6, a Reactor Scram was automatically initiated when PRM Channel 6 detector cabling shorted due to insulation degradation. This degradation is typical of detectors that have been in service greater than one year. Installed new detector and cabling for PRM Channel 6 and retested sat. The reactor was subsequently refueled and returned to operation.

fype Cause 4/23/03 1159 RRI Blade 'D' Not In Contact With Magnet On April 23, a Rod Not In Contact With Magnet Rod Run-In was automatically initiated when Blade 'D' anvil separated from its magnet. The Blade 'D' Full In Light was lit so the Console Watch initiated a manual scram and the reactor was shut down. Investigation revealed that the anvil was leaning on the north wall of the upper housing and that the drive itself was not securely fastened to the upper housing. The upper housing was realigned and the drive was securely fastened to the upper housing. The drive was retested as part of startup checks. The reactor was subsequently refueled and returned to operation.

TMe Cause 4/29/03 1160 Scram Apparent Airlock Door failure On April 29, a Reactor Scram was manually initiated when control room personnel observed that both containment airlock doors were open at the same time on the facility security video system. The Master Control Switch was placed to the OFF position securing the reactor. Investigation revealed that the failure was not in the containment airlock doors but rather with the video system. The video feed had frozen while the doors were cycling giving the appearance both inner and outer airlock doors were open at the same time. Containment integrity was never lost. The reactor was subsequently returned to operation.
