Dear Dr. Tokuhiro:
This letter forwards to you personal privacy information associated with the operator licensing examinations administered during the week of March 10, 2003. This information supplements the publicly available information in the examination report. Enclosure 1 lists individual grades for all candidates. Enclosure 2 includes individual candidate summaries for all candidates (License/Denial Letters and supporting documentation). Due to items noted in the Examination Report, sent under separate cover letter, I am requesting that you and your training staff personnel review the individual Examination Reports (NRC forms 301-N-1) in detail for assistance in improving your training program.
