Submittal of Collective Significance Review of the Causal Factors Associated with the RPV Head Degradation and Submittal of Revision 2 of the Management and Human Performance Improvement Plan
Dear Mr. Dyer:
On January 9, 2003 (Serial 1-1299), the FirstEnergy Nuclear Operating Company (FENOC) submitted several root cause analysis reports and assessments that were performed to thoroughly investigate and address organizational, programmatic and human performance issues that may have contributed to the RPV head degradation at Davis-Besse. In accordance with the commitment in Serial 1-1299, FENOC is submitting the Collective Significance Review of the Causal Factors Associated with the Reactor Pressure Vessel Head Degradation at Davis-Besse. This Collective Significance Review provides the results of a collective evaluation of the root cause analysis reports and assessments associated with the RPV head degradation to provide an integrated, collective review of identified causes associated with organizational or managerial issues, and look for any underlying factors or global issues not identified, or not identified as being significant by the individual sources alone.
In addition, on January 9, 2003 (Serial 1-1300), FENOC submitted Revision 0 of the Management and Human Performance Improvement Plan dated September 5, 2002. This plan has subsequently been revised to incorporate additional root cause analysis report and assessment results performed since the original revision was issued. FENOC is providing Revision 2 of the Management and Human Performance Improvement Plan as an enclosure to this letter.
