-,*There are templates for performing "phase 2" of the "significance determination process" that assigns a "color" to inspection findings. However, those are not applicable to all findings and circumstances When there is a finding (e.g., a hole in the D-B head) that doesn't fit any existing "phase 2" template, the process goes to "phase 3" which is completely free-form.
